Comparison of the Eradication Rates According to the Duration of PBMT Therapy as a Second-line Treatment for Helicobacter pylori Infection

Abstract
Background/Aims: Even with the currently most effective treatment regimen, i,e. proton pump inhibitor (PPI)-based triple therapy, around 10 to 20% of patients will fail to achieve the eradication of Helicobacter pylori (H. pylori) infection. As a second-line treatment in case of the first-line failure, proton pump inhibitor-based quadruple therapy (proton pump inhibitor, bismuth preparation, metronidazole, tetracycline: PBMT) is recommended, but the optimal duration of the treatment is still unclear. We retrospectively analysed the eradication rate of H. pylori according to the duration of the second-line PBMT therapy. Methods: Between January 2003 and October 2004, patients who failed to respond to primary eradication therapy, received one or two week PBMT therapy. Four to six weeks after the completion of medication, 13C-urea breath test was performed to detect H. pylori. Results: Eighty-five patients received quadruple therapy and whose mean age was 49.3 years (46 males and 39 females). Forty-six patients (PBMT1 group) received the treatment for one week and thirty-nine patients (PBMT2 group) received it for two weeks. The two groups has no difference in terms of mean age, sex, smoking and underlying disease. The eradication rate of PBMT1 group was 95.6% (44/46) and that of PBMT2 group was 89.7% (35/39). There was no significant difference of the eradication rate of H. pylori according to the duration of treatment. Conclusions: Seven days based PPI-based quadruple therapy may be adequate enough as the second-line treatment for the eradication of H. pylori.
