헬리코박터 파일로리와 기타 소화기 질환

Abstract

Helicobacter pylori (H. pylori) is the most prevalent bacterial pathogen that infects approximately half of the world’s population. It is well known that H. pylori causes chronic gastritis, peptic ulcer, and gastric cancer. H. pylori can also be associated with other gastrointestinal diseases. Epidemiologic studies have reported an inverse correlation between H. pylori infection and gastroesophageal reflux disease. However, the results of H. pylori eradication in patients with gastroesophageal reflux disease were not consistent with epidemiologic studies. Many studies have reported symptomatic improvement in uninvestigated dyspepsia and functional dyspepsia (FD) following H. pylori eradication, suggesting that H. pylori is the cause of dyspepsia. Therefore, it has been suggested that H. pylori-associated dyspepsia should be considered an entity distinct from FD and only the patients with persistent symptoms following eradication should be considered as having FD. A majority of studies also support an association of H. pylori with colon neoplasms and an inverse correlation with inflammatory bowel disease. However, no association of H. pylori with irritable bowel syndrome has been reported.

서 론

헬리코박터 파일로리는 전 세계 인구의 반 이상이 감염되어 있으며, 최근 국내 다기관 연구에서도 우리나라 헬리코박터 파일로리 유병률은 51%로 높은 감염률을 보였지만 과거에 비하여 감소되었다[1]. 헬리코박터 파일로리는 만성 위염, 소화성 궤양, 위 변연부 B세포 림프종 그리고 위암의 원인인자로 규명되었다[2]. 또한 이처럼 명확한 원인인자로서 밝혀진 질환 외에 헬리코박터 파일로리는 다른 소화기 질환과도 연관성이 제기되고 있다. 대표적으로 위식도역류질환과 기능성 소화불량증이 있으며, 이들 질환에서 헬리코박터 파일로리와 연관성에 대하여 가장 많은 연구가 이루어졌다. 여러 역학 연구에서 헬리코박터 파일로리는 위식도역류질환과 역의 상관관계를 보였고[3], 기능성 소화불량증에서는 헬리코박터 파일로리 제균 치료 후 6개월 이상 증상 호전을 보이는 경우 헬리코박터 파일로리를 소화불량증을 유발하는 기질적 원인으로 간주하는 헬리코박터 파일로리 연관 소화불량증(Helicobacter pylori-associated dyspepsia)이라는 개념이 최근 도입되었다[4]. 또한 헬리코박터 파일로리는 장내 미생물의 변화와 고가스트린혈증, 전신적 면역 반응의 조절 등을 통하여 대장암이나 염증성 장질환 및 과민성 대장 증후군에도 영향을 줄 수 있어 이들 질환과 연관 가능성이 제기 되었다[5-7]. 본고에서는 헬리코박터 파일로리가 명확한 원인인자로 규명된 위 질환을 제외한 다른 소화기 질환 중에 위식도역류질환, 기능성 소화불량증, 대장암, 염증성 장질환 및 과민성 대장 증후군에 대한 연구 결과를 살펴보고 헬리코박터 파일로리와의 연관성에 대하여 알아보고자 한다.

본 론

1. 위식도역류질환

헬리코박터 파일로리와 위식도역류질환의 연관성에 대해서는 아직 논란이 되고 있다. 여러 역학 연구에서 헬리코박터 파일로리는 위식도역류질환과 역의 상관관계를 보여주었다. 국내에서 건강검진 목적으로 위내시경과 헬리코박터 파일로리 혈청 검사를 시행한 5,616명을 대상으로 한 환자-대조군 연구에서 헬리코박터 파일로리는 위식도역류질환과 역의 상관관계를 보였으며, 위식도역류질환이 중증일수록 감염률은 감소되었다[3]. 11,690명을 대상으로 한 국내 설문지 연구에서도 헬리코박터 파일로리는 역류성 식도염의 위험도를 의미 있게 감소시켰다[8]. 서양의 역학 연구에서도 이러한 역의 상관관계가 보고되었다[9,10]. 또한 헬리코박터 파일로리는 바렛식도와 식도선암과도 역의 상관관계를 보여 헬리코박터 파일로리가 이들 질환의 발생을 예방할 수 있을 것이라고 주장하였다[11-13]. 이는 헬리코박터 파일로리 제균 치료 후 위염이 호전되면서 위산 분비를 증가시켜 위식도역류질환을 악화시키는 것으로 설명되고 있다. 그러나 위식도역류질환에서 제균 치료를 시행한 연구들에서는 다른 결과들을 보여주었다. 일부 연구에서는 제균 치료 후 위식도역류질환이 악화되었으나[14,15] 다른 연구에서는 오히려 제균 치료 후 위식도역류질환이 호전되었거나[16,17] 영향을 주지 않는다고 보고하였다[18]. 국내 12개 기관이 참여하였던 전향적 연구에서도 헬리코박터 제균 치료가 위식도역류질환의 악화와 발생에 영향을 주지 않았다고 보고하였다[19]. 위식도역류질환에서 헬리코박터 파일로리를 제균 치료한 연구들을 메타분석한 결과들도 서로 다른 결과를 보여주었다. 5개의 메타분석 중 1개의 메타분석에서는 헬리코박터 파일로리 제균 치료 후 위식도역류질환이 증가되었고, 특히 아시아인에서 유의하게 증가되었다고 보고하였지만[20], 다른 4개의 메타분석에서는 제균 치료 이후에도 위식도역류질환을 발생시키거나 악화시키지 않았다고 보고하였다(Table 1)[21-24]. 따라서 현재 국내 가이드라인에서는 헬리코박터 파일 제균 치료가 위식도역류질환의 발생 및 임상 경과에 영향을 미치지 않는다고 명시하고 있다[25]. Maastricht Consensus Report에서도 제균 치료가 위식도역류질환을 유발하거나 악화시키지 않기 때문에 위식도역류질환 환자에서 제균 치료를 해야 하는 경우 치료하도록 권고하고 있다[26].
정리해보면 역학 연구에서는 헬리코박터 파일로리와 위식도 역류질환 및 바렛식도, 식도선암이 역의 상관관계를 보여 헬리코박터 파일로리가 이들 질환에 대한 예방적 효과가 있을 것으로 여겨지지만, 제균 치료 후 위식도역류질환의 임상 경과를 본 대부분의 연구에서는 제균 치료 이후 위식도역류질환이 발생되거나 악화되지 않았다. 따라서 헬리코박터 파일로리와 위식도 역류질환의 연관성에 대해서는 아직 논란이 되고 있으며, 향후지역에 따른 유병률의 차이, 헬리코박터 파일로리 독성인자의 유전적 다형성 및 숙주 감수성 등을 고려한 양질의 연구가 더 필요하다.

2. 기능성 소화불량증

기능성 소화불량증과 헬리코박터 파일로리는 다른 소화기 질환에 비하여 비교적 명확하게 연관되어 있다고 알려져 있다. 기능성 소화불량증과 헬리코박터 파일로리의 연관성을 본 연구들을 메타분석하였을 때, 헬리코박터 파일로리와 기능성 소화불량증은 교차비 1.6 (95% 신뢰구간, 1.4~1.8)으로 의미 있는 연관성을 보였다[27]. 또한 헬리코박터 파일로리 제균 치료 이후에 기능성 소화불량증의 호전을 보인 연구들은 헬리코박터 파일로리가 기능성 소화불량증을 일으키는 원인임을 보여주었다. 국내에서 Rome III 기준으로 진단된 기능성 소화불량증 환자 213명을 대상으로 시행된 단일기관 연구에서 헬리코박터 파일로리가 제균된 경우 1년 후 소화불량증과 위염 증상이 호전됨을 보고하였다[28]. 최근 국내에서 시행된 전향적 다기관 연구에서도 성공적인 제균 치료가 1년 후 기능성 소화불량증의 호전과 연관되어 있음을 보고하였다. 하지만 이 연구는 제균되지 않았던 환자 수가 너무 적다는 제한점이 있다[29]. 기능성 소화불량 증에서 제균 치료 후 12개월간 추적 관찰한 14개의 무작위 대조 연구를 메타분석한 결과에서 헬리코박터 파일로리 제균 치료는 기능성 소화불량증 환자의 증상을 의미 있게 호전시켰다고 보고하였으며 이는 지역에 상관없이 일치하는 결과를 보였다[30]. 코크란 리뷰에서도 헬리코박터 파일로리 제균 치료는 효과가 크지는 않았지만(필요 치료 환자 수=15) 기능성 소화불량 증 증상을 의미 있게 호전시켰다[31]. 따라서 서구의 가이드라인에서는 소화불량증의 초기 접근 방법으로 경고 증상이 없는 경우 헬리코박터 파일로리를 먼저 검사한 후 양성인 경우 치료하도록 하는 헬리코박터 파일로리 검사 및 치료(test and treat) 방법을 권고하고 있다[26,32]. 이는 초기에 내시경을 시행하는 것과 비교하여 비용 효과적이면서 증상의 호전 정도는 비슷하였던 연구 결과들에 의한 것이다. 하지만 우리나라처럼 헬리코박터 파일로리의 유병률이 높고 위암의 발생률이 높으며, 특히 젊은 연령에서도 위암이 발생하는 국가에서는 이와 같이 내시경검사를 생략하고 헬리코박터 제균 치료만 할 경우 위암과 같은 악성 질환을 놓칠 가능성이 있다. 또한 항생제 내성 균주 증가의 위험성 및 제균 치료 약제에 의한 부작용 등을 고려하여 조심스러운 접근이 필요하다. 따라서 국내 가이드라인에서는 소화불량증 환자에서 초기 접근 방법으로 헬리코박터 파일로리검사 및 치료 방법보다 초기에 내시경을 권고하고 있다[25].
헬리코박터 파일로리가 소화불량증을 유발하는 병태생리는 명백하게 밝혀진 바는 없지만 위배출 장애와 같은 위장관 운동장애나 내장 과민성의 증가, 위산 분비능의 변화, 위십이지장점막의 비만세포나 호산구의 침윤, 장내 미생물 무리의 변화 등이 제시되고 있다[33-35]. 2015년 Kyoto Consensus Report는 헬리코박터 파일로리 위염을 기질적 감염성 위염으로 분류하고 헬리코박터 파일로리 연관 소화불량증이라는 개념을 제시하여 기능성 소화불량증과 다른 기질적 소화불량증으로 분류하였다[4]. 다시 말하여 헬리코박터 파일로리 제균 치료 이후에도 6개월 이상 소화불량증의 호전이 있는 경우에는 헬리코박터 파일로리 연관 소화불량증으로 정의하였으며, 제균 치료에도 증상의 호전이 없는 경우에만 기능성 소화불량증으로 정의하도록 하였다. 이러한 개념은 개정된 Rome IV 진단기준에도 적용되어 헬리코박터 파일로리 감염을 기질적 소화불량증의 아형으로 분류하였다[36].
요약하면 헬리코박터 파일로리는 기능성 소화불량증의 원인이 되며, 제균 치료를 통하여 소화불량증 증상을 유의하게 호전시킬 수 있는 것으로 보인다. 최근에는 제균 치료 이후 소화불량증이 호전되는 경우 헬리코박터 파일로리를 소화불량증의 기질적 원인으로 간주하고 있다. 따라서 서구의 가이드라인에서는 소화불량증의 초기 접근 방법으로 헬리코박터 파일로리 검사 및 치료 방법을 권고하고 있다. 하지만 헬리코박터 파일로리와 위암의 유병률이 높은 우리나라에서 이러한 권고안을 그대로 받아들일지에 대해서는 우리 실정에 맞는 연구가 필요하다.

3. 대장암

헬리코박터 파일로리와 대장암과의 연관성에 대해서는 많은 연구가 이루어져 왔고, 헬리코박터 파일로리가 대장암이나 대장 선종의 발생에 연관되었을 가능성을 보고하였다. 건강검진 목적으로 대장내시경과 위내시경을 시행한 4,446명을 대상으로한 국내의 코호트 연구에서 헬리코박터 파일로리 감염 환자에서 진행성 대장 선종과 다발성 대장 선종이 1.8배 정도 더 많았다[37]. 건강검진 환자 8,916명을 대상으로 한 우리나라 단면 연구에서도 헬리코박터 파일로리 감염 환자에서 진행성 대장 선종이 2배 정도 더 많았다[38]. 한편 우리나라의 후향성 단면 연구에서는 건강검진으로 내시경을 시행한 6,351명의 환자 중 위축성 위염이 동반된 헬리코박터 파일로리 감염 환자에서는 진행성 대장 선종 및 대장암이 1.4배 정도 증가하였으나 위축성 위염이 동반되지 않은 헬리코박터 파일로리 감염 환자에서는 증가되지 않았다[39]. 보통 위축성 위염은 헬리코박터 파일로리의 만성적인 감염의 결과로 발생되는 것을 감안할 때 오랜 기간의 헬리코박터 파일로리 감염이 대장암이나 전암 병변의 발생에 관여됨을 유추해볼 수 있다. 우리나라와 헬리코박터 파일로리 유병률이 비슷한 중국에서 최근에 시행된 전향적 연구에서도 헬리코박터 파일로리 감염 환자에서 대장 선종의 발생이 3.2배 증가됨을 보고하였다[40]. 헬리코박터 파일로리 유병률이 낮은 미국의 연구에서는 다양한 결과를 보여주었다. 헬리코박터 파일로리 혈청 분석을 이용한 8,000명 정도의 코호트 내 환자-대조군 연구에서 헬리코박터 파일로리 감염이 대장암을 증가시키지는 않았지만 vacuolating cytotoxin A (VacA) 양성 균주인 경우 대장암의 위험도를 11% 증가시켰고, VacA 항체가 높을수록 위험성이 더 증가하였다. 특히 아프리카계 미국인과 아시아계 미국인에서 이러한 현상이 뚜렷하였고, 백인에서는 연관성을 보이지 않았다[41]. 인종에 따른 차이는 인종에 따른 VacA 변이의 다양성과 숙주 면역 반응의 다양성으로 설명하고 있는데, 아프리카계 미국인 혹은 아시아계 미국인에서 백인보다 활성화된 VacA가 높게 나타난다고 한다. 대부분 백인 환자를 대상으로 한 다른 연구에서도 헬리코박터 파일로리는 대장암의 발생과 연관성을 보이지 않았지만[42] 15만명을 대상으로 한 연구에서는 헬리코박터 파일로리 위염 환자에서 대장 종양의 발생이 많았으며, 특히 진행된 선종이나 대장암의 발생은 2배 정도 증가됨을 보여주었다[43]. 2013년에 시행된 27개의 연구에 대한 메타분석에서 헬리코박터 파일로리 감염은 대장 선종과 대장암의 발생을 각각 1.66배, 1.39배 증가시키는 것으로 보고하였고, 성별과 지역에 따른 차이는 없었으며, 헬리코박터 파일로리를 진단하는 방법에 따른 차이는 있었다[44]. 최근 PubMed 검색으로 헬리코박터 파일로리와 대장암의 연관성을 분석한 연구[45]에서는 40개의 환자-대조군 연구(아시아 연구 22개) 중 30개의 연구가 헬리코박터 파일로리가 대장암과 연관됨을 보고하였고, 교차비는 1.15에서 10.6으로 다양하였다. 6개의 전향적 연구(아시아 연구 1개)에서는 3개의 연구에서 교차비 1.11~2.24로 연관성을 보여주었다.
위의 결과를 종합해볼 때 헬리코박터 파일로리는 대장암이나 대장 선종의 발생을 증가시키는 것으로 판단된다. 아시아에서는 연관성이 비교적 뚜렷하고 서구에서는 백인을 제외한 다른 인종에서는 연관성이 있고, 특히 VacA 양성 균주인 경우 높은 연관성을 보인다. 대부분이 환자-대조군 연구여서 향후 더 좋은 디자인의 연구들이 필요하지만 헬리코박터 파일로리 감염 환자, 특히 위축성 위염 등이 동반된 경우에는 대장암 예방을 위하여 더 엄격한 관리가 필요할 것으로 생각된다.
헬리코박터 파일로리가 대장암을 유발하는 기전은 명확하지 않지만 여러 가지의 가능성을 고려해볼 수 있다. 첫째, 헬리코박터 파일로리와 헬리코박터 파일로리에서 분비되는 독소가 직접적으로 대장 조직에 영향을 줄 수 있다. 실제로 몇몇 연구에서 PCR이나 조직검사를 통하여 대장 종양에서 헬리코박터 파일로리가 존재함을 보여 주었고, 대조군보다 대장 종양 조직에서 헬리코박터 파일로리가 많이 발견되었다[46,47]. 둘째, 장내 미생물의 변화로 대장암을 유발할 수 있다. 헬리코박터 파일로리에 의한 위 병변이 있는 환자에서 장내 미생물의 변화를 보여주었는데, 이러한 장내 미생물의 변화가 대장암의 발생과 연관될 수 있을 것이다.5 셋째, 가스트린의 증가가 대장암을 발생시킬 수 있다[6,48,49]. 소규모의 전향적 환자-대조군 연구에서 헬리코박터 파일로리나 cytotoxin-associated gene A보다 고가스트린혈증이 대장 선종과 유의하게 관련되어 있었다[48]. 그 밖에 헬리코박터 파일로리에 유발되는 대사 질환이나 전신적인 염증반응 등이 대장암의 발생에 기여하였을 것으로 여겨지고 있다.

4. 염증성 장질환

염증성 장질환은 면역 기능 이상, 유전적 감수성, 장내 미생물 무리의 변화, 환경적 요인 등이 복합적으로 작용하여 발병한다. 최근 많은 역학 연구와 실험 연구들은 염증성 장질환과 헬리코박터 파일로리가 역의 상관관계가 있음을 보고하였고, 헬리코박터 파일로리가 염증성 장질환 발생에 방어적 역할을 할 것으로 제시하였다. 국내 전향적 다기관 공동 연구에서 헬리코박터 파일로리 감염률은 염증성 장질환 환자와 대조군에서 각 각 25.3%, 52.5% (P <0.001)로 염증성 장질환 환자에서 헬리코박터 파일로리 감염률이 의미 있게 낮았다[50]. 특히 크론병 환자에서 17.7%로 더 낮은 감염률을 보였고, 60세 이하에서 이러한 연관성이 더욱 뚜렷하였다. 미국의 데이터베이스를 이용한 65,515명의 연구에서도 헬리코박터 파일로리 감염은 크론병과 궤양성 대장염 환자에서 각각 0.48배, 0.59배로 낮게 나타나 역의 상관관계를 보였고, 반대로 헬리코박터 파일로리 음성 위염은 크론병과 궤양성 대장염 환자에서 각각 11.06배, 2.25배로 높게 나타나 양의 상관관계를 보였다[51]. 염증성 장질환과 헬리코박터 파일로리와의 연관성을 본 많은 연구들 중 일부 소규모의 연구를 제외한 대부분의 연구에서 대조군에 비하여 크론병이나 궤양성 대장염 환자에서 낮은 헬리코박터 파일로리 감염률을 보고하였다[52]. 3개의 메타분석의 결과는 헬리코박터 파일로리가 염증성 장질환의 발생을 억제한다는 것을 더욱 명확하게 보여주었다(Table 2)[53-55]. 이 메타분석들은 연구들 간의 이질성과 출판 편견이 높은 제한점이 있었지만 이러한 제한점을 극복한 메타분석에서도 헬리코박터 파일로리와 염증성 장질환 간의 역의 상관관계가 있음을 보여주었다[56].
헬리코박터 파일로리 제균 치료가 염증성 장질환을 발생시키거나 악화시킬 수 있는지에 대한 증거는 아직 부족하다. 두 개의 증례 보고에서 제균 치료 이후에 새로 발생된 크론병에 대한 보고가 있었다[57,58]. 하지만 코호트 연구에서는 6명의 크론병 환자에서 제균 치료 이후에 Crohn’s disease activity index, CRP, fecal calprotection의 변화는 없었다고 보고하였다[59].
헬리코박터 파일로리에 의한 염증성 장질환의 억제 효과에 대한 기전은 전신적인 면역 관용과 염증 반응의 억제에 의한 것으로 간주된다. 수지상 세포(dendritic cell)는 적응 면역계(adaptive immunity system)를 조절하는 중추적인 역할을 한다. 실험 연구에서 헬리코박터 파일로리에 의하여 수지상 세포는 기능적으로 면역 관용의 표현형을 가지는 반성숙(semi-mature) 상태가 되는 것으로 나타났다[7,60]. 이러한 면역 관용의 특성이 자가면역의 병적인 면역 반응을 억제할 것으로 여겨진다. 조절 T세포(regulatory T cell)는 효과 T세포(effector T cell)의 분화를 억제하고 자가면역 질환의 억제 및 면역 항상성 유지에 필수적인 것으로 알려져 있다. 지속적인 헬리코박터 파일로리 감염은 면역 억제 조절 T세포의 분화를 유도하는 것으로 나타났고, 림프구 재순환 등을 통한 전신적인 면역 조절로 염증성 장질환에서 면역 반응을 억제할 것으로 여겨진다[61,62]. 또한 transforming growth factor-β, interleukin-10, nucleotide-binding domain, leucine-rich-containing family, pyrin domain-containing-3, interleukin-18 등이 헬리코박터 파일로리가 염증성 장질환을 억제하는 데 있어 중요한 역할을 하는것으로 알려져 있다[63-65].
정리해보면 헬리코박터 파일로리는 대부분의 연구에서 염증성 장질환과 역의 상관관계를 보이고, 이는 헬리코박터 파일로리가 염증성 장질환의 발생을 억제할 수 있는 가능성을 나타낸다. 헬리코박터 파일로리에 의한 전신적인 면역 관용과 염증 반응의 억제를 통하여 이러한 예방적 역할을 하는 것으로 제시되고 있다.

5. 과민성 장증후군

헬리코박터 파일로리와 과민성 장증후군의 연관성에 대한 연구는 많지 않다. 국내 연구는 아직 보고된 바가 없으며, 외국의 연구에서도 건강한 사람에 비하여 과민성 장증후군 환자에서 헬리코박터 파일로리 감염률이 높았다는 연구[66,67]와 크게 다르지 않다는 연구가 혼재되어 있다[68]. 또한 설사형 과민성 장증후군에서 헬리코박터 파일로리 제균 치료 이후에 증상 변화를 본 연구에서도 유의한 증상 변화는 보이지 않았다[66,69]. 최근 9개의 환자 대조군 연구를 메타분석한 결과에서도 과민성 장증후군환자에서 헬리코박터 파일로리 감염률이 약간 높긴 하였으나 통계적인 유의한 차이를 보이지 않았으며 결과들의 이질성도 높았다[70]. 따라서 헬리코박터 파일로리와 과민성 장증후군의 연관성은 낮아 보이지만 연구가 많지 않아 향후 제균 치료 이후 과민성 장증후군의 임상 경과에 대한 더 많은 연구가 필요하다.

6. 만성 변비

장내 미생물 무리의 변화가 과민성 장증후군이나 만성 변비의 병태생리의 원인으로 제시되고 있다. 헬리코박터 파일로리도 이러한 장내 미생물 무리에 영향을 줄 수 있지만 대부분 위내 미생물 무리에 대한 연구이다. 분변에서 16S rRNA 유전자분석을 이용한 일본의 코호트 연구에서 헬리코박터 파일로리에 감염된 환자가 비감염자에 비하여 Actinomyces, Gemella, StreptococcusHaemophilus가 증가되었고, 특히 심한 위축성 위염이 있는 환자에서 Streptococcus가 의미 있게 증가되었다[71]. 그러나 독일의 연구에서는 헬리코박터 파일로리 감염환자에서 Lactobacillus 가 증가하였고[72], 중국의 연구에서는 헬리코박터 파일로리가 Lactobacillus를 억제한다고 보고하여[73] 연구마다 상이한 결과를 보여 주었다. 최근 만성 변비 환자에게 제균 치료 후 증상 변화를 본 연구에서 헬리코박터 파일로리감염 환자에서 만성 변비의 유병률이 일반인에 비하여 높았고(48.8% vs. 28.4%), 제균 치료 2개월 후 치료에 실패한 환자에서는 변비 증상의 호전이 없었으나 성공한 환자에서는 만성 변비의 증상이 의미 있게 호전되었다[74]. 저자들은 제균 치료 후 위산 분비능의 회복과 장내 미생물의 변화가 만성 변비 증상의 호전에 기여하였을 것으로 주장하였다. 그러나 만성 변비에서 제균 치료에 대한 다른 연구가 없어, 향후 만성 변비나 과민성장증후군에서 제균 치료 전후 증상의 호전 정도와 장내 미생물 무리의 변화에 대한 연구가 필요하다.

결 론

헬리코박터 파일로리는 명확한 원인인자로 밝혀진 만성 위염, 소화성 궤양, 위 변연부 B세포 림프종 및 위암 외에도 다른 소화기 질환에도 영향을 줄 수 있다(Table 3). 헬리코박터 파일로리는 기능성 소화불량증이나 대장암, 대장 선종의 발생을 증가시키며, 염증성 장질환과는 역의 상관관계를 보여 염증성 장질환의 발생을 억제할 것으로 여겨진다. 위식도역류질환은 많은 연구들이 있었지만 역학 연구와 제균 치료 후 증상 변화를 본 연구 결과들이 상반된 결과를 보인다. 과민성 장 증후군에서는 연구가 많지 않으며 결과도 일치하지 않으나, 만성 변비에서는 연구가 제한적이지만 제균 치료가 증상을 호전시킨다. 하지만 헬리코박터 파일로리의 지역에 따른 유병률의 차이, 독성인자의 유전적 다형성 및 숙주 면역 반응의 다양성 등이 연구 결과들에 영향을 줄 수 있다는 점을 고려하여 이해해야 한다. 특히 우리나라는 헬리코박터 파일로리와 위암의 유병률이 높기 때문에 국내 현실에 적용하기 위한 연구가 필요하다.
