Barrett’s esophagus (BE) is caused by metaplasia of squamous epithelium into columnar epithelium, mainly in the lower esophagus. Updates have been made in the etiology, diagnosis and treatment of BE since its first report as an ulcerative lesion in the lower esophagus. Columnar metaplasia of the lower esophagus has been reported as a result of gastroesophageal reflux, rather than genetic factors. When specialized intestinal metaplasia (SIM) is confirmed on biopsy, it is diagnosed as BE; otherwise, it is described as columnar line esophagus (CLE). Recent recommendations state that the following two conditions must be satisfied to diagnose BE: CLE with a length ≥1 cm proximal to the gastroesophageal junction, or SIM, with goblet cells confirmed on biopsy. BE is a key risk factor for esophageal adenocarcinoma. Early detection and treatment of BE, dysplasia, and early esophageal adenocarcinoma arising from BE has become a critical issue in the USA and Europe. The prevalence of BE in Korea is likely to increase because of the increasing prevalence of gastroesophageal reflux diseases. However, there are few reports on the diagnosis and treatment of BE in Korea. In this narrative review, important historical discoveries related to BE, the anatomical structures and endoscopic findings around the gastroesophageal junction required for diagnosing CLE and BE, and a brief review of the guidelines for the diagnosis and treatment of BE are summarized.
Norman Barrett
이후 Adler는 원주상피로 대체된 식도 상피의 후천적인 변화의 원인으로서 다음과 같은 3가지 가능성을 제시하였다: 1) 흉강 내로 위의 이동(upward extension from the stomach), 2) 편평상피세포의 선상피로의 화생(metaplasia from squamous epithelium), 3) 식도 상피 표층에 존재하고 있던 원주상피(from the superficial cardiac glands of the esophagus).

Conception of the anatomy of the esophago-gastric junction, suggested by Hayward. The imaginary line XY separates the esophagus and the stomach; the upper part is defined as the esophagus and the lower part is defined as the stomach. A to C is lined by squamous epithelium, C to E is lined by junctional epithelium while F is lined by fundal epithelium. The cardia is defined from the point B, where the phreno-esophageal ligament begins, to the point D, above the imaginary line XY. The cardia is lined partly by squamous, and partly by columnar epithelium, and thus the junctional epithelium extends across the esophago-gastric junction.
(A) The gastroesophageal (GE) junction, distal end of palisade zone, proximal margin of gastric fold and squamo-columnar junction are all on the same plane. In this case, the Z-line cannot be checked with the video endoscope at the first entry. (B) When the patient is instructed to perform the Valsalva maneuver, the Z-line moves upward and is observed above the narrowest part of the esophageal lumen; this is defined as pinch-cock action. (C) The solid black line is the Z-line, which represents the squamo-columnar junction, while the area indicated by the solid red line is where the esophageal lumen is narrowed by the pinch-cock action. (D) If the proximal part of the gastric mucosal fold is connected with an imaginary line, a solid blue line can be drawn. In most studies, the GE junction is defined by this line. (E) When the distal end of the palisade zone of the esophagus is connected with an imaginary line, we can draw a solid green line. This line can be used as an auxiliary line to define the GE junction. (F) Though this example falls under the normal category, the GE junction, distal end of palisade zone, proximal margin of gastric fold and squamo-columnar junction may not be perfectly aligned.
(A) In columnar line esophagus (CLE), salmon-colored columnar epithelium and subepithelial vessels can be observed. (B) When the proximal end of the gastric mucosal fold is defined as the gastro-esophageal (GE) junction, the esophagus between the Z-line and the solid blue line corresponds to CLE. (C) When the distal end of the palisade zone is defined as the GE junction, (D) the esophagus between the Z-line and the solid green line corresponds to CLE. However, in all situations, the minimum length of CLE must be ≥1 cm to satisfy the diagnostic criteria for Barrett's esophagus.
(A) The Z-line corresponding to the gastro-esophageal (GE) junction is observed proximal to the pinchcock action (PCA). Assuming the PCA is the same as the location of the diaphragm, the gastric mucosa is located in the chest cavity. In this case, hiatus hernia is also seen. (B) Barrett's esophagus is observed in the proximal region, accompanied by erosive esophagitis.
The key steps are: 1. Identify the gastro-esophageal (GE) junction at the top of the gastric mucosal folds. 2. If hiatus hernia is present, do not confuse the diaphragmatic hiatal impression for the GE junction. 3. For circumferential columnar-appearing mucosa above the GE junction, define the extent in centimeters above the GE junction – report as the C value. 4. For any tongue-like areas of columnar-appearing mucosa, measure the maximum extent, in centimeters, above the GE junction – report as the M value. UI, upper inscisor.
