Endoscopic Findings of Kyoto Classification of Gastritis

Abstract

Several studies have conclusively established an association between upper gastrointestinal diseases such as gastric cancer and Helicobacter pylori (H. pylori) infection; thus, it is important to assess H. pylori infection based on endoscopic findings. The Kyoto classification of gastritis is a classification that comprehensively describes the association between an individual’s H. pylori infection status and endoscopic findings. Characteristic endoscopic findings in uninfected individuals include a regular arrangement of collecting venules, fundic gland polyps, and red streaks, among other such features. Characteristic endoscopic findings in patients with current H. pylori infection include diffuse and spotty mucosal erythema, atrophy, intestinal metaplasia, enlarged or tortuous folds, secretion of sticky mucus, mucosal nodularity, foveolar hyperplastic polyps, and/or xanthomas. Characteristic endoscopic findings in previously infected individuals include patchy and map-like mucosal erythema. This classification can reflect the risk of gastric cancer and can benefit primary care physicians, as well as expert endoscopists owing to its easy applicability in routine clinical practice.

서 론

위염의 내시경 진단은 1922년에 Schindler [1]가 위경을 이용하여 내시경 위염의 존재를 보고한 뒤 Schindler의 위염 분류를 바탕으로 여러 가지의 위염 분류가 제시되었으며, 위축의 범위 진단에 대해서는 Kimura-Takemoto 분류가 이용되어 왔다[2]. 1983년 Warren과 Marshall이 Helicobacter pylori (H. pylori) 균을 발견하고 H. pylori 감염이 위염의 원인이 될 수 있다는 것이 밝혀진 후 1990년에 원인, 조직 소견, 내시경 소견을 체계화한 Sydney system이 제창되었다[3]. 그 후 조직학적 분류는 updated Sydney system으로 개편되어 국제적인 분류로서 지위를 확립하였다[4]. Sydney system은 국제적으로 통용되어 왔지만, 내시경 분류에 있어서는 몇 가지 문제점이 있어 일반 임상에서는 널리 보급되지 못하였다.

2013년 일본에서는 H. pylori 위염에 대한 제균 치료의 보험 적용 확대로 H. pylori 제균 시대를 맞이하게 되었으며[5,6], 또한 내시경 기기의 발달로 조직학적 위염을 어느 정도 내시경으로 진단할 수 있게 되었다. 그러므로 H. pylori 위염을 중심으로 하여 세계에 통용될 수 있는 새로운 위염 분류의 제창이 필요하게 되었으며, 이러한 배경을 토대로 2013년 일본소화기내시경학회에서 새로운 위염 분류에 관한 주제를 토의하게 되었고, 학회 종료 후에도 계속 심의되어 "위염의 Kyoto 분류"가 제창되었다[7]. 이에 본고에서는 Kyoto 분류에서 제시하고 있는 위염의 내시경 소견에 대하여 살펴보고자 한다.

본 론

위염의 Kyoto 분류는 위점막의 상태를 H. pylori 미감염 점막, H. pylori 현감염 점막 및 H. pylori 기감염(제균 후) 점막으로 나누어 진단하기 위하여 내시경 검사에서 관찰되는 19개의 위염 소견을 정의하여 각각을 H. pylori 감염 상태에 따라 분류하고 있다(Table 1) [7]. 예를 들면 regular arrangement of collecting venules (RAC), 미만성 발적(diffuse redness), 지도상 발적(map-like redness)은 각각 H. pylori 미감염, H. pylori 현감염, H. pylori 기감염(제균 후)의 특징적인 소견이다. Sydney system에서 사용되는 내시경 소견과의 차이점은 미만성 발적, 지도상 발적, 장상피화생(intestinal metaplasia, IM), 백탁 점액(sticky mucus), 다발성 백색 편평 융기(multiple white flat elevated lesion), RAC가 새로운 내시경 소견으로 Kyoto 분류에 추가되었고, 반면 삼출물(exudate)과 점막 취약성(friability)은 Kyoto 분류에서 제외되었다.

1. H. pylori 감염에 따른 내시경 소견

1) H. pylori 미감염 내시경 소견(Fig. 1)

H. pylori 미감염의 위저선 점막에는 규칙적으로 배열하는 미세한 붉은 점들이 관찰된다. 이 소견을 RAC라고 하며, 정상 위점막에서 보이는 소견이다. 염증이 없는 위저선 점막에서는 원형의 선관 구조가 배열하고 선관 주위를 에워싸는 미세 혈관들이 벌집 모양의 네트워크를 형성한다. 이러한 미세 혈관들이 융합하여 집합세정맥(collecting vein)이 되고 점막하층의 정맥으로 이어진다. 내시경 검사 시 집합세정맥이 위점막 표면에 투영되는 형태가 RAC이다. 전정부 근위부와 위체부에서 RAC가 관찰된다면 H. pylori 미감염 점막을 시사한다. RAC는 멀리서는 미세한 붉은 점들로 인식되지만 근접 시 불가사리 형태의 혈관들을 확인할 수 있다. 전정부에 국한된 H. pylori 감염 위염에서는 위체부 점막에 RAC가 관찰될 수 있으므로, H. pylori 미감염과 현감염 여부를 감별하기 위해서는 RAC의 판정은 위각부와 하부 체부 소만에서 실시하는 것이 좋다[8]. 한편 유문선 점막에서는 선관의 구조 형태가 다르기 때문에 H. pylori 미감염에서도 RAC는 보이지 않는다.

위저선 용종(fundic gland polyp)은 일반적으로 H. pylori 미감염 또는 H. pylori 제균 후 관찰되며, 위저선 용종의 존재는 H. pylori 음성을 시사한다. 특히 양성자펌프억제제(proton pump inhibitor)의 장기 복용에 의하여 위저선 용종의 발생이 증가될 수 있다. 헤마틴(hematin), 선상 발적(red streak), 융기형 미란(raised erosion)은 H. pylori 미감염자에서 좀 더 많이 관찰되지만, H. pylori 감염자에서도 보일 수 있으므로 이러한 소견만으로는 H. pylori 미감염을 단정지을 수는 없다.

2) H. pylori 현감염 내시경 소견(Fig. 2)

H. pylori 감염 시 염증을 동반한 위점막에서는 점막표층에 존재하는 모세 혈관의 울혈 및 확장에 의하여 점막 전체가 발적을 띄게 되는데, 이를 미만성 발적이라고 한다. 이러한 점막의 색조 변화는 hemoglobin index가 H. pylori 위염일 때 상승하는 것과 비슷한 의미이다. 또한 미만성 발적으로 인하여 좀 더 깊은 곳에 위치한 집합세정맥이 점막 표면에서 관찰되지 않게 되는데, 이를 RAC의 소실이라고 한다.

H. pylori 감염이 장기간 지속되면 고유위선의 감소로 인한 위축성 변화가 발생하여 내시경 검사 시 다양한 정도의 점막 주름의 위축, 혈관 투견 등의 위축 소견이 나타난다. 장상피화생은 위축이 진행함에 따라 나타나며, 내시경 검사 시 회백색조의 편평 융기가 관찰되면 장상피화생에 특이적인 소견이라 할 수 있다. 하지만 장상피화생이 이렇게 관찰되는 경우는 일부에 불과하며, 정확한 진단을 위해서는 methylene blue 염색이 유용하다고 알려져 있으나 우리나라에서는 거의 사용되지 않는다. 최근에는 위점막의 융모상 변화나 indigo carmine 색소내시경에서 위소구의 크기와 모양이 불규칙한 경우가 장상피화생 진단의 유용한 지표가 된다고 알려져 있다[9].

H. pylori 감염과 관련된 기타 내시경 소견으로는 점막 종창(mucosal edema), 위체부 주름의 종대(enlarged fold)와 사행(tortuous fold), 위저선 영역의 점상 발적(spotty redness), 백탁 점액, 황색종(xanthoma) [10], 닭살 모양의 결절성 변화(nodularity) [11], 선와상피-과형성성 용종(foveolar-hyperplastic polyp) [12]이 있다. 반상 발적(patchy redness)과 함몰형 미란(depressive erosion)은 H. pylori 감염 상태와 관련 없이 나타나는 소견으로, 비스테로이드성 항염증 약물(non-steroidal anti-inflammatory drug) 등으로 인한 점막 손상과 관련이 많다.

3) H. pylori 기감염(제균 후) 내시경 소견(Fig. 3)

H. pylori의 제균 치료가 이루어지면 위점막의 염증이 개선된다. 조직학적으로 제균 성공 직후 다핵구 침윤은 저명하게 개선되어 거의 사라지며, 단핵구 침윤도 제균 직후 크게 감소하고 남아 있는 단핵구 침윤은 제균 후 4~5년에 걸쳐 점진적으로 개선된다. 그러므로 제균 후 내시경 소견의 변화는 제균 당시의 위염의 상태와 관찰 시기에 따라 차이가 있다. 따라서 제균 후에는 H. pylori 감염 시 내시경 소견과 H. pylori 미감염 시 내시경 소견이 같이 관찰되는 경우가 흔하다. 제균 성공 후 단기간 내에 점상 발적, 미만성 발적, 점막 종창, 위주름의 종대와 사행, 백탁 점액은 개선되는 경우가 많다[13]. 한편 제균 후 장기 경과 시에는 RAC가 회복되거나 일부에서 위축, 장상피화생도 개선되기도 하지만 개선의 속도와 정도는 개인차가 있다.

제균 후 특징적인 내시경 소견으로는 지도상 발적이 있다. H. pylori 양성인 만성 위축성 위염에서는 흔히 혈관이 투영되는 위축 영역이 퇴색ㆍ백색으로, 위저선이 남아 있는 비위축 영역이 발적으로 관찰된다. 하지만 제균 후에는 위축 영역의 장상피화생을 가진 점막이 발적으로, 반대로 비위축 영역의 위저선 점막은 미만성 발적이 소실하기 때문에 백색조로 나타나게 된다. 이처럼 제균 후 나타나는 위축 영역의 발적을 지도상 발적으로 부르며, 이 선 경계에서의 색조의 역전은 현저하여 색조역전현상(reversed phenomenon on mucosal borderline)이라고 하기도 한다.

지도상 발적, 색조역전현상의 존재가 확인된 경우나 위축 점막과 동시에 미감염 소견인 RAC와 위저선 용종이 관찰되는 경우에는 제균 후 점막이라고 진단하기에는 별로 어려움이 없다. 하지만 위축 점막에서는 부정형의 집합세정맥이 관찰될 수 있어서 제균 후의 RAC 회복을 평가할 때에는 주의가 필요하다.

2. 위암 위험도를 고려한 내시경 소견 점수화 체계

내시경 소견(위축, 장상피화생, 주름의 종대, 닭살 모양의 결절)을 토대로 그리고 H. pylori 감염과 H. pylori 제균 후를 구별하는 소견으로 미만성 발적을 포함시켜[14] 위암의 위험도를 예측할 수 있는 점수화 체계를 구성할 수 있다(Table 2) [7]. 즉, 제균에 의하여 염증세포 침윤이 소실되면 위점막이 미감염 위점막에 가깝게 회복되므로 내시경 소견을 통하여 위암의 위험도가 감소됨을 예측할 수 있다.

1) 위축(A)

내시경 검사 시 위축은 혈관투영상으로 판정을 하며 위축의 정도와 범위는 충분히 공기를 주입하여 관찰한다. Kimura-Takemoto 분류를 사용하여 기술하며[7] 위축은 유문부에서 시작하여 체부 소만, 이후 체부 전체로 확장하므로 범위에 따라 C-1, C-2, C-3, O-1, O-2, O-3의 6단계로 구분한다. C는 close를, O는 open을 의미하며, 분문부부터 유문부까지 위축이 연결되지 않은 경우를 close type, 연결된 경우를 open type이라고 분류한다.

위축이 관찰되지 않는 C-0, C-1은 점수 0점, 위축이 경한 C-2, C-3는 점수 1점, 위축이 중정도부터 심한 경우인 O-1~O-3은 점수 2점으로 계산한다. 예를 들면 점수 1점의 경우에는 A₁으로 기재한다.

2) 장상피화생(IM)

장상피화생은 일반 백색광 관찰 및 영상강화내시경(image-enhanced endoscopy, IEE)을 이용한 관찰에서 다른 소견을 보인다. 백색광으로는 특이형 장상피화생이라고 불리는 백색융기 병변과 제균 후 나타나는 발적 함몰 병변의 관찰만 가능하다. 그러나 methylene blue를 사용한 색소내시경에서는 특이형 이외의 장상피화생도 청색으로 염색되어 관찰된다. 협대역 영상(narrow band imaging, NBI)에서 선와상피의 표면에 청백색의 선이 보이는 light blue crest (LBC)가 관찰되면 장상피화생으로 진단할 수 있다. 또한 특이형의 장상피화생에서는 NBI로 관찰 시 점막 상피에 백색 물질이 부착되어 있는 white opaque substance (WOS)를 볼 수 있다. IEE 관찰에서는 이러한 LBC, WOS의 정도와 범위를 평가한다. 따라서 백색광 관찰 및 IEE 관찰 소견을 구분하여 기재한다.

장상피화생이 관찰되지는 않는 경우는 점수 0점, 장상피화생이 전정부에 국한되어 있는 경우는 점수 1점, 장상피화생이 체부까지 확장되어 있는 경우는 점수 2점이다. 백색광 관찰에서 점수 1점인 경우는 IM₁으로 기재한 뒤 IEE 관찰 소견은 괄호 내에 표기한다. 백색광 관찰 점수 1점, IEE 관찰 점수 2점의 경우에는 IM₁₍₂₎로 기재한다.

3) 주름의 종대(H)

충분한 공기를 주입하여 관찰 시 주름의 폭이 4 mm 이하인 경우는 점수 0점, 5 mm 이상인 경우는 점수 1점이며 H₁으로 기재한다.

4) 닭살 모양의 결절(N)

닭살 모양의 결절이 관찰되지 않는 경우는 점수 0점, 관찰되는 경우는 1점이며 N₁이라고 기재한다.

5) 미만성 발적(DR)

위축이 없는 체부선 영역을 관찰한다. 숙련되지 않는 경우에는 체부선 영역의 집합세정맥의 관찰 여부를 토대로 미만성을 평가하는 것이 좋다. RAC가 관찰되는 경우는 미만성 발적이 없으므로 점수 0점, RAC가 소실된 경우는 점수 2점, 제균 후와 같이 일부에서 RAC가 관찰되는 경우는 점수 1점으로 한다. 점수 2점인 경우는 DR₂로 기재한다.

6) 전체 점수

모든 인자를 A₁ IM₁₍₂₎ H₀ N₁ DR₂로 기재하며, 전체 최종 점수(IEE 점수는 계산하지 않음)는 괄호 내에 기입하여 A₁ IM₁₍₂₎ H₀ N₁ DR₂₍₅₎로 기재한다. 전체 점수는 최소 0점에서 최대 8점이다.

3. 내시경 소견 기재 방법

Kyoto 분류의 내시경 기재 과정은 다음과 같다[15].

1) H. pylori 감염 위염(현감염, 활동성 위염), H. pylori 감염 병력, H. pylori 미감염을 구분하고, 국제적으로 통용하기 위하여 영어로 기재한다. 현감염의 경우는 active-gastritis, 기감염은 inactive-gastritis, 미감염은 non-gastritis로 기재한다.

2) 이후 위축 범위(Kimura-Takemoto 분류)를 괄호 안에 기재한다. 예를 들면, active-gastritis (O-1)로 기재한다.

3) 위염의 진단에 사용한 소견 이외의 필요한 소견에 대해서는 다음에 기재한다. 또한, 각 내시경 소견의 부위를 of 다음에 기재한다. 예를 들면, active-gastritis (C-3) with erosion of antrum, A₁ IM₁ H₀ N₀ DR₂₍₄₎로 기재한다.

결 론

위염의 Kyoto 분류는 내시경 시 관찰되는 소견을 구체적으로 정리하고 이러한 소견을 위암의 가장 중요한 원인 인자인 H. pylori 감염 여부와의 연관성을 포함한 분류이다. 실제 국내에서 위내시경 검사의 주목적이 위암의 조기 발견이므로, 이러한 Kyoto 분류는 이전 분류보다는 위내시경 검사 시 좀 더 많은 정보를 가지고 체계적으로 접근할 수 있는 길을 제시해줄 것으로 생각된다. 또한 이 중 중요한 내시경 소견을 토대로 점수화한 위암의 위험도 점수 체계는 추후 임상 연구를 통하여 검증이 필요하지만, 스크리닝 내시경 검사 기간의 선별에도 어느 정도 도움을 줄 수 있지 않을까 하는 기대를 가져 본다.
