Trans Abstract

Since its discovery, Helicobacter pylori (H. pylori) has played a central role in the pathogenesis of gastroduodenal diseases. Additionally, there have been many studies that demonstrated the role of H. pylori infection in extragastric diseases. Many research studies have confirmed an indirect association between the prevalence of H. pylori infection and the occurrence of cardiovascular diseases (CVDs); however, the results are still conflicting. Most studies found that the involvement of H. pylori in this process was related to the chronic inflammation. This inflammation may facilitate the development of CVD-related atherosclerotic pathologies such as endothelial damage and chronic activation of coagulation cascade, etc. Furthermore, H. pylori infection is known to be associated with other vascular diseases such as cerebrovascular diseases and cardiac arrhythmias, especially atrial fibrillation. This review summarized the literature on the association of cardiovascular manifestations and H. pylori infection and provided information about the pathogenesis of this association.

서 론

많은 역학적 연구에도 불구하고 아직 일치된 결론을 얻지는 못하였지만 헬리코박터 파일로리(Helicobacter pylori, H. pylori) 감염이 심혈관 질환과 관계가 있을 것이라는 것은 널리 알려진 이야기 중의 하나이다. H. pylori 감염과 심혈관 질환의 연관성에 대한 연구 결과는 긍정적인 견해와 부정적인 견해가 맞서고 있어 명확한 결론을 내기는 힘든 상태이며, 많은 역학연구에서 여러 병인론이 제안되고 있지만 다양한 교란 변수에 대한 완전한 배제가 이루어지지 않고 병인론을 뒷받침할 수 있는 증거들이 아직 부족한 실정이다. 본고에서는 H. pylori 감염과 심혈관 질환의 연관성을 현재까지 나온 문헌 고찰을 통하여 정리해보고자 한다.

본 론

1. H. pylori 와 관상동맥 질환

1) 최근까지의 연구 동향

1994년 Mendall 등[1]이 H. pylori 감염과 관상동맥 질환(coronary artery disease, CAD)의 관련성에 대하여 처음 제안한 이후 많은 연구들이 진행되어 왔다. H. pylori 감염과 CAD 발생과의 연관성을 살펴본 연구[2-21]는 양자 간 연관이 있다고 보고한 연구[4-6,9,10,13-15,17-21]와 양자 간 연관이 없다고 보고한 연구[2,3,7,8,11,12,16]가 맞서고 있으며 아직까지 명확한 결론을 도출하지 못하고 있다(Table 1). 이러한 이유는 연구 방법 및 연구 대상의 이질성 때문으로 생각되며, 교란 변수를 배제하였는지, 연구 대상이 건강인인지 심장 질환 환자인지 등에 따라 결과가 다르게 나타날 가능성이 있다[22,23].

최근의 메타분석에서는 관상동맥 조영술로 진단된 CAD와 H. pylori 감염 간의 상관성을 보고하였는데, H. pylori에 감염되었을 경우 CAD의 위험도가 더 높았다(OR: 1.42, 95% CI: 1.09~1.86; P =0.008). 이러한 현상은 60세 미만의 인구에서 더 뚜렷하였고 유럽과 미국의 연구에서 더 유의한 상관성을 보여 나이와 인종이 관계가 있는 것으로 나타났다[18]. 다른 메타분석에서도 CAD 및 심근경색과 H. pylori 감염의 연관 교차비는 2.53 (95% CI: 1.37~4.67)이었으며 통계적으로 유의한 의미가 있었다[19]. H. pylori와 CAD과의 관계에 대한 최근의 대만 연구에서는 제균 치료를 시행한 군에서 CAD와의 관련성 감소(2.58% vs. 3.35%; P=0.09), CAD 관련 사망률의 저하(2.86% vs. 4.43%; P <0.01)를 보여주었다[21]. 또한, multidetector computed tomography로 측정한 관상동맥경화증과 H. pylori 감염 양성군과의 연관성이 보고되기도 하였다(OR: 2.18, 95% CI: 1.051~7.528; P=0.04) (Table 1)[20].

Cytotoxin-associated gene A (CagA) 양성 H. pylori 균주와 CAD와 관련성에 대한 연구들도 많이 이루어졌다. 현재까지의 연구 결과들을 살펴보면 CagA 양성 H. pylori 감염은 심혈관 질환의 이환율을 현저히 증가시키는 것으로 보인다[24]. 심근경색 환자, 불안정 협심증 환자에서 CagA 양성 H. pylori 감염률이 대조군에 비하여 현저히 높았다는 연구 결과가 있다(심근경색 86.7%, 불안정 협심증 91.7%, 대조군 58.3%)[25]. 또 다른 연구에서는 관상동맥경화가 있는 환자군에서 CagA 항체가 월등히 높게 측정되었으며, CagA 항체의 역가가 높을수록 동맥경화 혈관의 길이가 길었다는 것이 특이할 만한 점이었다[26]. Huang 등[27]은 CagA 양성 H. pylori에 감염되었을 때 CRP, 총콜레스테롤, 저밀도지질단백질, 산화된 저밀도지질단백 그리고 아포지질단백질이 CagA 음성 H. pylori 에 감염된 경우에 비하여 유의하게 증가된다고 보고하였다. 하지만 CagA 양성 H. pylori 감염이 심혈관 질환의 위험인자임을 밝히지 못한 연구들[7,16,28,29]도 있어 이 역시 일치된 결론을 도출하지 못하고 있다. 현재까지의 연구를 종합해보면 CagA 양성 H. pylori 감염이 혈전 형성을 유도하여 동맥경화가 훨씬 더 일어나기 쉬운환경을 조성하는 것으로 생각된다[22,24] H. pylori 제균 치료 후 CAD의 호전 여부를 관찰한 연구에서는 혈관내막 기능의 개선[30] 및 재발이 줄었다는 연구 보고[31]가 있는 반면 CAD의 재발에 큰 차이가 없다는 연구 보고도 있다[32].

사망률과 관련해서는 최근의 메타분석에 따르면 건강한 무증상 피험자에서 H. pylori 제균 치료는 암 사망률 감소와 관련이 있지만 전반적인 사망률은 증가 경향을 보이는 것으로 보고되었다[33]. 그러나 Kim 등[34]은 고혈압 치료 환자를 포함한 국민건강보험 데이터베이스를 이용한 인구 기반 연구에서 다른 결과를 보고하였다. H. pylori 제균 치료를 받은 5,541명의 환자와 제균 치료를 받지 않은 1,182명 환자의 사망률을 비교하였을 때, 전체 사망률 감소(hazard ratio: 0.70, 95% CI: 0.60~0.82) 또는 뇌혈관 사망률(hazard ratio: 0.46, 95% CI: 0.26~0.81)의 감소를 나타냈으나 반대로 심혈관 질환으로 인한 사망률은 차이가 없었다[34].

2) 심혈관 질환과 연관된 병태생리

H. pylori는 혈액 내의 단핵구를 자극하여 혈액 응고를 유발시킬 수 있으며, von Willebrand factor 항원은 H. pylori 감염과 강한 상관성이 있다. H. pylori는 plasminogen activator inhibitor-1의 발현을 자극하고 혈소판의 활성 및 응집을 일으키며, 중성지방 및 다양한 동맥경화인자를 증가시킨다. 또한, H. pylori는 혈청 tumor necrosis factor-alpha 수치 상승과 연관이 있다[35]. 만성 H. pylori 감염 상태에서는 저등급의 급성염증 반응과 피브리노겐(fibrinogen)과 같은 급성기 응고 단백질이 증가된다. 이러한 기전을 통하여 급성 혈전성 혈관 질환이 발생할 수 있는데 fibrinogen은 H. pylori 감염과 매우 연관이 있는 것으로 보고되고 있다[36]. 또한, 만성 염증 상태에서는 관련된 사이토카인 등이 분비되고 이 영향으로 혈중 지질 조성 변화, 혈액 응고 체계의 활성화, 혈소판 응집 유도 및 혈관 내피층의 손상을 유발하여 동맥경화가 더 잘 유발되는 것으로 생각하고 있다(Fig. 1)[22,36,37].

2. H. pylori 와 뇌혈관 질환

동맥경화의 유발인자로서 H. pylori 감염이 연구되면서 허혈성 뇌혈관 질환과 H. pylori 감염의 연관성도 연구되었다. 과거에 시행된 일부 소규모 연구에서는 H. pylori 감염이 동맥경화와 연관된 미세혈관병 중 뇌혈관 질환과 관련이 있다고 하였으나[38-42] 대규모 연구에서는 H. pylori 감염이 허혈성 뇌혈관 질환과 연관이 없거나 약간 영향이 있는 정도로 밝혀졌다[5]. CagA 양성 H. pylori가 허혈성 뇌혈관 질환의 병태생리에 미치는 영향에 대해서도 보고된 바 있는데, Diomedi 등[43]에 따르면 CagA 양성 H. pylori 감염이 있는 경우 허혈성 뇌혈관 질환의 재발률이 월등이 높았다는 것이 밝혀졌다. 또한, 10개의 환자-대조군 연구를 분석한 메타분석에서는 CagA 양성 H. pylori에 감염된 경우 CagA 음성 H. pylori 감염군보다 허혈성 뇌혈관 질환의 위험도가 2.66배 높다고 보고하였다[13]. 또 다른 메타분석에서도 H. pylori CagA 혈청 양성은 허혈성 뇌졸중의 위험과 밀접한 관련이 있어 CagA 혈청 양성은 다른 진단 도구보다 뇌졸중의 위험을 보다 효과적으로 예측할 수 있다고 보고하였다[44]. 하지만 전향적 코호트 연구에서는 다른 결과가 도출되었다. 미국인을 대상으로 시행된 전향적 코호트 연구에서는 H. pylori 양성군에서 허혈성 뇌질환의 OR를 1.13 (95% CI: 0.68~1.89)으로 보고하였으나, 이 연구에서는 CagA 균주와 뇌졸중의 아형을 고려하지 않은 단점이 있다[45]. ESTHER 코호트를 이용한 또 다른 전향적 연구에서는 뇌졸중 위험, H. pylori 혈청 양성 및 CagA 혈청 양성 사이의 연관성을 조사하였으나 CagA 음성 또는 CagA 양성 H. pylori는 뇌졸중 위험에 예측효과가 없었다[16].

3. H. pylori 와 부정맥

H. pylori 감염과 심장 부정맥의 관계는 1998년 처음 제시되었고, 당시 Frustaci 등[46]은 염증에 의한 심방 구조 변화를 제시하여 심방세동(atrial fibrillation, AF)과 염증 반응이 관계가 있음을 발표하였다. 2001년에 Chung 등[47]은 AF와 혈액 내 염증인자와의 관련성에 대하여 발표한 이후, 2005년 Montenero 등[48]은 AF 환자에서 대조군보다 H. pylori 항체 및 CRP가 높음을 증명하여 H. pylori 감염이 AF의 원인이 될 수 있음을 제시하였다. 이들은 위세포의 양성자펌프인 H+/K+-ATPase와 심장세포의 펌프인 Na+/K+-ATPase 간에 유사성이 있다는 점을 고려하여 H. pylori 감염자들의 일부에서 관찰되는 위벽세포의 자가항체가 심장의 ATP 가수분해를 저해함으로 이온의 항상성에 불균형을 초래하여 심방세동을 초래하였을 것이라는 기전을 제시하였다[48]. 이후 H. pylori 감염과 부정맥, 특히 심방세동과의 관계를 살펴본 여러 연구가 있었지만 H. pylori 감염이 심방세동과 연관이 있고 심방세동의 발생에 영향을 미친다는 연구가 있는 반면, 심방세동과는 관련이 없다는 연구도 있어 아직까지 일치된 결과를 보이지 못하고 있다(Table 2)[48-55]. 최근 Tetta 등[56]은 심방세동과 관련하여 6개의 후향적 관찰 연구를 포함한 메타분석을 수행하였으나 H. pylori 유병률은 AF 그룹과 비AF 그룹(각각 35.0% 및 32.7%) 사이에 차이가 없었다.

결 론

H. pylori 와 심혈관 질환의 연관성에 대해서는 아직도 논란이 있다. 심혈관 질환 발생에 영향을 주는 위험인자는 매우 다양하여 보정하여 분석하기에는 어려운 점이 많다. 많은 연구에서 결과들이 일치하지 않는 것은 보정해야 할 교란 변수들이 너무 많고 복잡하다는 점에 기인한다. 현재까지의 근거로는 H. pylori 는 만성 염증 반응을 통하여 동맥경화의 기전에 관여하며, 특히 CagA 양성 H. pylori 균주는 혈전 형성을 유도하여 동맥경화를 훨씬 더 잘 일으킬 수 있다는 것이다. 우리나라는 H. pylori 감염과 심혈관 질환 두 가지의 유병률이 모두 높으므로 이들의 연관성을 규명하는 것은 질병의 예방 및 치료를 위하여 매우 의미 있는 일이라 할 수 있겠다. 따라서 향후 H. pylori 와 심혈관 질환의 인과관계를 규명하기 위해서는 전향적인 대규모 무작위 연구 및 H. pylori 제균 치료가 심혈관 질환의 예방에 어떤 영향을 미치는지에 대한 전향적 무작위 중재 실행 연구가 필요할 것으로 사료된다.
