Trans Abstract

Serum pepsinogen (PG), anti-Helicobacter pylori (H. pylori) immunoglobulin G (IgG), and gastrin-17 (G-17) are plasma biomarkers for gastritis. H. pylori serology titers and PG levels increase during active H. pylori infection; moreover, elevated PG II levels indicate a high risk for diffuse-type gastric cancer in East Asian populations. Serum PG I/II ratios and PG I levels decrease with the progression of gastric corpus atrophy; thus, a combination of serum PG I levels ≤70 ng/mL and a PG I/II ratio ≤3 (serologic atrophy) indicates a high risk of intestinal-type gastric cancer. Serum G-17 is often not used as an indicator in H. pylori-seroprevalent populations because it is usually elevated in subjects with H. pylori infections. When H. pylori is eradicated, most patients show a rapid decrease in serum PG II levels and anti-H. pylori IgG titers within a few months. Seroreversion is required for several months to years after regression of H. pylori. Moreover, seroreversion may not always be achieved in all eradicated cases. The serum PG I/II ratio starts to increase after eradication; therefore, serologic atrophy improves accordingly, unless severe atrophy is present. Thus, some eradicated patients may show normal serum assay findings but have a higher risk for developing gastric cancer than H. pylori-naive subjects. Furthermore, serum PG levels decrease after gastrectomy and increase with the intake of certain drugs (e.g., aspirin or acid suppressants) or in renal failure patients. Due to such wide variations, serum assays are inadequate for the confirmation of H. pylori eradication. It is useful when interpreted with gastroscopy and other H. pylori test findings.

서 론

위염을 진단하기 위해서 임상에서 처방할 수 있는 대표적인 혈청 검사로는 펩시노겐(pepsinogen, PG), 가스트린(gastrin), 항헬리코박터 파일로리(Helicobacter pylori, H. pylori) 항체 검사가 있다. H. pylori 감염률이 낮은 국가에서는 혈청 PG, gastrin-17 (G-17), 항H. pylori immunoglobulin G (IgG) 수치로 구성된 GastroPanel® (Biohit Oyj, Helsinki, Finland)로 위내시경 검사가 필요한 위험군을 선별한다[1]. 그러나 감염률이 높은 국가에서는 H. pylori 감염으로 인해 전정부의 가스트린 분비가 증가되어 G-17보다 PG 수치가 위암의 고위험군을 감별하는 데 유용하다[2]. 그러므로 우리나라와 일본에서는 GastroPanel® (Biohit Oyj) 대신 H. pylori에 대한 IgG와 PG 검사로만 구성된 ABC 분류법이 활용되고 있다. ABC 분류법은 혈청 PG I 수치가 70 ng/mL 이하이면서 PG I/II 비가 3.0 이하인 경우를 혈청학적 위축(serologic atrophy, gastric corpus atrophy)으로 진단한 뒤, 혈청 항H. pylori IgG 결과와 혈청학적 위축 유무로 위암에 대한 위험군을 감별하는 방법이다[3].

PG는 펩신의 전구체로서 위저선(oxyntic gland)내 주세포(chief cell)에서 주로 생성되므로, H. pylori 균이 위저선을 자극하여 벽세포(parietal cell)에서 위산 분비가 상승하면 PG 수치도 증가하고, 위축이나 장상피화생 변화로 인해 위저선이 손상되는 위산 저하 상태에서는 PG 수치도 감소한다[4]. 따라서 혈청 PG 검사는 위점막 분비능을 측정하기 위한 목적으로도 사용되고 있다. 본문에서는 혈청 PG 수치를 중심으로 H. pylori 감염 시와 제균 치료 시에 관찰되는 소견에 대해 살펴보고, 한국인에서 위암 선별 검사로 사용할 때의 주의사항과 ABC 분류법의 한계점에 대해서 알아보고자 한다.

본 론

1. H. pylori 감염 시의 혈청 검사 소견

1) H. pylori에 대한 혈청 IgG 수치의 증가

혈청 항H. pylori 항체 검사 시에는 반드시 IgG 수치를 확인해야 하며 IgM이나 IgA 수치는 정확도가 낮아서 임상에서는 사용하지 않는다[5]. 항체 수치는 균의 양과 비례하므로 IgG 수치가 높을수록 현감염일 가능성이 높고, 수치가 낮을수록 과거 감염이 나 미감염일 가능성이 높다[6]. 예를 들면, 혈청 항H. pylori IgG 수치가 <5~>200 AU/mL로 보고되는 Chorus H. pylori IgG (DIESSEDiagnostica Senese, Siena, Italy)키트로 검사할 경우, 양성 항체가(≥12~>200 AU/mL)의 하위 25%인 60.0 AU/mL 미만은 위양성일 가능성이 높다[7]. 대부분의 활동성 감염자는 상한치에 가까운 높은 IgG 수치를 보이므로, 만약 수치가 낮다면 13C-요소호기 검사(urea breath test, UBT)나 단클론성 대변항원 검사(monoclonal stool antigen test)로 감염 여부를 확인해야 한다[8]. 또한, H. pylori 균주가 지역마다 달라서 해당 지역에서 검증된 IgG 검사만을 사용해야 하는데, 현재 국내에서 시판되는 혈청 항H. pylori IgG 제품 중에서 한국인에서의 정확도가 논문으로 보고된 것은 5종류에 불과하다(Table 1). 침습적인 H. pylori 검사 결과를 기준으로 할 때, 국산 제품인 Genedia H. pylori IgG (Green Cross Medical Science, Eumsung, Korea) 검사의 민감도는 92.3~100%, 특이도는 81.3~83.5%로[9,10], 비침습적인 검사인 13C-UBT 결과를 기준으로 할 때의 민감도 100%, 특이도 80.7%와 유사하였다[7]. 외국 제품 중 H. pylori IgG Immulite 2000 (Siemens Healthcare Diagnostics Products Ltd., Los Angeles, CA, USA) 검사와 H. pylori IgG (Radim, Rome, Italy) 검사의 한국인에서의 정확도는 국산 제품과 유사하였다[11,12].

Validated Serum Anti-H. pylori IgG Assays in Korean Adults Using Enzyme-Linked Immunosorbent Assay (ELISA) Method

혈청 항H. pylori 항체 검사는 위양성률이 높다는 점, 지역별로 검증된 키트만 사용해야 한다는 점, 수개월이 지나서야 감염 상태가 수치로 반영된다는 점 등의 제한점이 있지만 무증상 감염자를 진단하는 데 도움이 된다[13]. 본원에서 H. pylori 감염이 없는 혈청항체 음성자 407명을 57.7±21.4개월간 추적검사한 결과, 61명(15.0%)에서 혈청전환을 보였는데 그중에서 새로운 H. pylori 감염이 진단된 6명(9.8%)은 IgG 수치가 6.5 AU/mL에서 132.7 AU/mL로 유의하게 증가된 소견을 보였다. 반면에 혈청전환은 되었지만 H. pylori가 진단되지 않은 위양성자 55명은 IgG 수치가 6.4 AU/mL에서 22.0 AU/mL로 낮은 상승을 보였으며, 1~2년 후에 다시 혈청음전되는 경향을 보였다. 그 결과, 혈청전환은 해마다 3.3%씩 발생할 정도로 흔했지만 실제로 감염된 사람은 9.8%에 불과하였다. 또한, H. pylori 감염이 진단된 6명은 모두 기저부에서 점상 발적(spotty redness)이 새로 관찰되었고, 평균 혈청항체 수치는 감염 전에 비해 20.4배 상승해서 감염자를 감별하는 데 도움이 되었다[13].

2) 혈청 PG II 수치의 증가

PG I은 기저부와 체부에서만 생성되지만 PG II는 위 전체와 십이지장의 구부에서 생성되므로[14], H. pylori 감염 시 혈청 PG I와 PG II 수치가 모두 증가한다[15]. 특히 PG I 수치가 85 ng/mL 이상이거나 PG II 수치가 15 ng/mL 이상이면 감염자일 가능성이 높고[16], PG I 수치가 100 ng/mL 이상이면 체부 우세형 감염을 시사한다[17]. 하지만 감염이 장기간 지속되어 spasmolytic polypeptide-expressing metaplasia (SPEM)가 발생할 가능성이 높아질수록 PG I 수치는 감소한다[18]. 이처럼 같은 감염자라도 위축성 위염이 심할수록 PG I 분비는 감소하고, 위축이 없으면 PG I 분비가 증가하므로 감염을 진단할 때는 PG I보다 PG II 수치가 유용하다.

혈청 PG II 수치가 증가하는 현상은 위암 발생률이 높은 H. pylori에 감염되었을 때만 관찰된다[19]. 감염 시 PG II 수치는 2배 이상 증가하며[20], 대부분의 감염자는 PG II 수치가 8.25 μg/L보다 높고 급성 위염이 있는 경우에는 10.25 μg/L보다 높다[21]. 체부 위염이 심할수록 PG II 수치가 증가하므로, 기능성 소화불량증 환자 중에서 PG II 수치가 높은 감염자는 제균 후에 증상이 호전될 가능성이 높다는 보고도 있다[22]. PG II 생성은 유전적 다형성(polymorphism)에 의해 조절되며 PG II 100 bp 위치의 다형성이 H. pylori 감염으로 인한 위암 발생과 연관된다[23]. 국내 연구에 의하면 PG II 수치가 20 ng/mL 이상으로 높은 H. pylori 감염자는 조기 미만형 위암이 발생할 위험성이 12.8배 높았으며, 40세 미만이거나 여성일수록 높았다[24]. 일본에서는 혈청 PG II 수치가 30 ng/mL 이상으로 높으면 미만형 위암이 발생할 위험성이 3.8배 높다는 보고가 있었으며[25], 여성에서의 위험성은 7.4배로 남성의 4.1배보다 높다는 보고도 있었다[26]. 하지만 2013년부터 모든 감염자를 제균하기 시작하면서 PG II 수치가 높은 활동성 감염자들이 미만형 위암으로 진행하기 전에 대부분 치료되어, 일본에서는 더 이상 PG II 수치가 높은 미만형 위암 환자에 대한 연구가 없다.

3) 혈청 PG I/II 비의 감소

H. pylori 감염이 체부와 기저부로 퍼질수록 PG II 수치는 증가하지만, 근위부의 위점막이 손상되기 시작하면 PG I 생성이 저하되어 PG I/II 비가 감소한다[27]. 따라서 혈청 PG II 수치가 높고 PG I/II 비가 낮을수록 감염자일 가능성이 높다. PG II 수치가 12 ng/mL를 초과하고 PG I/II 비가 4.5 미만일 경우, 90% 이상에서 감염이 진단되었으며[28], 감염이 있는 위암 환자는 PG II 수치가 15.9 μg/L보다 높고 PG I/II 비가 5.4보다 낮은 경향을 보인다[29].

감염이 지속되면 위암 발생률이 증가하므로[30], 감염자를 놓치지 않고 진단하는 것이 중요하다. 그러나 PG I//II 비가 낮은 만성 위염 환자들은 H. pylori 균이 불균일하게 분포하므로, 조직 검사 시 위음성으로 나올 가능성이 높다[31]. 본원에서 872명 한국인들의 침습적 검사와 비침습적인 검사의 H. pylori 진단 일치율을 조사한 결과, 5명 중 1명은 위음성으로 나왔다[32]. 특히 위선종이나 선암이 진단된 조직에서는 위음성율이 11배나 높았는데, H. pylori가 위점막을 손상시킨 뒤에 자가소멸하는 hit & run 현상으로 해석된다. 따라서 혈청 PG I/II 비가 낮은 경우, 침습적인 검사에서 균이 검출되지 않아도 13C-UBT나 대변항원 검사를 추가해서 감염 여부를 확인하는 것이 바람직하다. 2021년 현재 적용되는 보건복지부 고시 제2017-263호 UBT 급여인정기준에 의하면 1, 2차 제균 치료 후나 출혈 경향이 높은 질환이 있는 소화성 궤양 환자가 아닌 경우에는 비급여로 처방해야 한다[33]. 또한, 보건복지부 고시 제2018-80호 H. pylori 검사 급여인정기준에 의하면 내시경으로 궤양이 확인된 경우나 저등급의 위 말트림프종, 조기위암 절제술 후, 특발성 혈소판감소성 자반증 환자가 아닌 경우에는 대변항원 검사는 본인부담 90%로 처방해야 한다[34].

2. 제균 성공 시의 혈청 검사 소견의 변화

1) 혈청 항H. pylori IgG 수치의 감소

혈청 IgG 수치가 >200 AU/mL로 높은 감염자도 제균에 성공하면 4개월 만에 78.2 AU/mL로 빠르게 감소할 수 있다(Fig. 1). 이 74세 남자의 경우, 제균 후에 체부의 점상 발적이 사라져서 제균 성공을 확인하는 데 도움이 되었다. 대부분의 감염자는 제균 3개월 후에 혈청 항H. pylori IgG 수치가 50% 이상 감소하지만[35], 위축이 심할수록 느리고 적게 감소하는 등 위점막 상태에 따라 항체의 감소폭과 기간이 다르다[36]. 제균 전과 6년 후의 혈청 항H. pylori IgG 수치와 cytotoxin-associated gene A에 대한 항체 수치를 비교한 연구에서는 위암의 발생률이 높을수록 감소폭이 적었다[37].

대부분의 감염자는 제균 후 18개월 안에 혈청 IgG 수치가 유의하게 감소하지만 혈청음전은 45%에서만 관찰된다[38]. 이처럼 제균 후 오랜 시간이 지나도 위양성 소견을 보이는 과거 감염자가 흔하므로, 혈청음전으로 제균 여부를 판정할 수 없다(Table 2). 혈청 IgG 수치는 위내시경 소견이나 다른 H. pylori 검사 결과가 의심스러울 때, 감염 상태를 판단하는 데 도움이 되지만 확진용 검사는 아니다.

Common Changes in Serum Assay Findings Observed after H. pylori Eradication

2) 혈청 PG II 수치의 감소

PG 검사 중에서 제균 성공 시 가장 뚜렷한 변화를 보이는 것은 PG II 수치의 감소이며[39], PG I/II 비의 증가와 PG I 수치의 감소가 그 뒤를 잇는다[40]. 제균 성공 시 8주 만에 PG II 수치가 22.7% 이상 감소한다. 위암 발생률이 높은 지역의 이탈리아 연구에서는 감염 시 9.93 μg/L 이상으로 증가했던 PG II 수치가 제균 후에는 25% 이상 감소하였다(민감도 93%, 특이도 91%) [41]. 327명을 분석한 국내 연구에서는 제균 후에 PG II 수치가 25.4 ng/mL에서 9.1 ng/mL로 감소하였는데, 위축성 위염이 심하지 않은 60세 미만의 감염자일수록 크게 감소하였다[42]. 이처럼 나이나 위점막 손상도에 따라 감소폭이 다르므로 PG II 수치는 혈청 IgG 수치처럼 제균 확진용으로 사용될 수 없다.

H. pylori 균이 사라진 후의 혈청 수치의 변화는 주로 24개월 안에 관찰된다[43]. PG I 수치가 3.2 ng/mL, PG I/II 비가 0.4로 혈청학적 위축이 진단된 감염자도 제균에 성공하면 2년 안에 혈청 PG 수치가 정상화될 수 있다(Fig. 2). 이 45세 여성의 경우, 제균 후 2년 만에 위점막의 혈관상이 정상 소견으로 회복되었다. 결절성 위염이 사라진 자리에는 전정부에 위축 경계가 있는 폐쇄형 1형 위축성 위염이 관찰되었다. 혈청 항H. pylori IgG 수치는 >200 AU/mL에서 9.4 AU/mL로, PG II 수치는 7.1 ng/mL에서 5.4 ng/mL로 각각 감소하여 재감염이 없음을 알 수 있었다.

제균 후에도 주름비대가 심해지고 PG II 수치가 증가한다면 제균 실패나 숨겨진 병변 등을 감별해야 한다. 13C-UBT 음성 결과로 제균 성공을 확인한 후에도 주름비대와 PG II 수치의 상승이 지속되어 복부 컴퓨터단층촬영 후에 전위절제술을 한 결과, 보만 4형의 진행성 위암이 확진된 62세 남자를 보고한 바 있다[44]. 제균 후에 혈청 항H. pylori IgG 수치는 61.4 AU/mL에서 46.0 AU/mL로 감소하는 경향을 보였으나, 추적 위내시경 검사에서 주름은 제균 전보다 딱딱하고 두꺼워졌으며 공기를 주입해도 펴지지 않았다. 환자는 제균 후에 다른 약물을 복용한 적이 없다고 했음에도 불구하고, PG II 수치가 15.5 ng/mL에서 16.8 ng/mL로 증가하고 주름비대가 악화되어 점막 하암을 의심했다. 수술 후 진단은 pT4aN3bM0 (stage 3C)의 인환세포형의 저응집형 암(poorly-cohesive carcinoma)으로, 위내시경 조직 검사로는 진단할 수 없는 보만 4형 위암이었다. 주름비대 소견으로 인해 위내시경 검사를 할 때마다 보만 4형이 강력히 의심되었으나, 추적 위내시경 검사에서도 암세포가 채취되지 않은 드문 진행성 위암이었다.

3) 혈청 PG I/II 비의 증가

PG I/II 비는 H. pylori 균이 사라진 후 12~15개월까지 서서히 회복된다[45]. 제균 후에 체부의 위축과 장상피화생이 호전되면서 관찰되는 현상으로, 만성 위염이 있는 조기위암 환자도 제균에 성공하면 PG I/II 비가 5년 안에 정상화될 수 있다[46]. 위축뿐만 아니라 장상피화생도 가역적인 변화를 보일 수 있으며[47], 내시경 소견으로도 확인할 수 있다(Fig. 3). 이 73세 남자의 경우, 제균 전에는 전정부와 체부에서 장상피화생이 진단되었는데, 제균 후에는 체부의 장상피화생이 호전되면서 PG I/II 비가 3.5에서 8.0로 회복되었다. 제균 후에도 전정부에서는 장상피화생이 진단되었지만, 체부까지 장상피화생이 있는 경우에 비해 위암 발생률은 낮다[48]. 유문선의 장상피화생은 SPEM과 무관하게 발생하며 제균 후에 호전되는 경우도 있지만[49], 고령의 남성은 여성에 비해 제균 후에도 회복되지 않을 가능성이 높다[50].

제균 성공 후의 장상피화생 호전 여부는 나이나 성별뿐만 아니라 언제 무엇을 어떻게 검사했는지에 따라 상이하므로, 추적 조직 검사에서 장상피화생이 남아있다는 이유만으로 비가역적이라고 섣불리 판단할 수 없다. 비가역성을 입증하기 위해서는 내시경적, 혈청학적, 조직학적 소견과 DNA methylation 분석 결과에서 제균 전후에 변화가 없어야 하는데, 장상피화생 환자에서 제균 후에 위 모두가 지속된다고 보고한 연구는 없다. 장상피화생 조직에서 관찰된 E-cadherin promoter hypermethylation는 제균 후 6주 안에도 호전되지만[51], 일부 oncogenic microRNA는 제균 1년 후에도 사라지지 않는다[52]. Interleukin-1β mRNA는 5년에 걸쳐 서서히 감소하므로[53], 가역성을 확인하려면 긴 시간이 필요하다. 한편, 16년간 장상피화생을 추적관찰한 연구에 의하면 불완전형에서만 위암의 위험성이 상승하였는데[54], 장상피화생의 아형에 대한 제균 전후의 차이를 보고한 연구도 부족하다. 또한, 장상피화생에 대한 조직검사는 주로 모양과 색조 변화가 가장 심한 부위에서 하므로, 채취된 조직이 위 전체를 대변하지 못한다. 따라서 제균 후에 화생성 위염의 범위가 줄고 혈청 PG 수치가 정상화되었거나 DNA methylation 변화가 보였다면 추적 조직 검사에서 불완전형 장상피화생이 사라질 가능성도 배제할 수 없으므로, 장기간 기다려야 한다.

개방형 위축성 위염과 혈청학적 위축도 제균 후에 호전될 수 있다(Fig. 4). 이 42세 여성의 경우, 제균 전에는 PG I 수치가 32.7 ng/mL, PG I/II 비가 2.2로 혈청학적 위축이 있었으며, 위축 경계가 분문부를 넘어선 개방형 위축성 위염 소견을 보였다. 그러나 제균 6년 후에는 체부의 위축이 사라지고 위축 경계가 전정부의 대만측에서 보이는 폐쇄형 1형 위축성 위염으로 호전되었다. PG I/II 비는 2.2에서 4.5로 증가하여 혈청학적 위축도 호전되었다.

H. pylori 균이 사라진 후에도 혈청학적 위축이 지속된다면 비가역적인 위점막 손상이 있다는 것을 시사한다[55]. 혈청학적 위축이 있던 감염자가 제균 치료 없이 혈청음전을 보였으나 혈청학적 위축이 지속된다면 균의 자가소멸을 의심해야 한다(Fig. 5). 이 52세 남자의 경우, H. pylori 감염 진단 당시의 PG I 수치는 17.7 ng/mL, PG I/II 비는 0.9로 혈청학적 위축이 있었다. 제균 치료를 원치 않았고 그 후에도 항생제를 복용한 적이 없다고 했으나, 8년 만에 항H. pylori IgG 수치는 195.5 AU/mL에서 9.2 AU/mL로, PG II 수치는 18.9 ng/mL에서 5.5 ng/mL로 각각 감소하여 자가소멸로 판단하였다. PG I 수치는 7.9 ng/mL, PG I/II 비는 1.4로 여전히 혈청학적 위축이 있었으며, 폐쇄형 3형 위축성 위염이 8년 만에 가장 심한 단계의 개방형 위축성 위염으로 진행되었다. 이처럼 균이 사라진 후에도 혈청학적 위축이 호전되지 않을 때 장형 위암과 위선종의 발생률이 가장 높다[56]. 나아가서 PG I/II 비가 3 이하이면 위암 절제 후에도 이시성 위암이나 선종이 발생할 가능성이 증가한다[57]. 하지만 혈청학적 위축만으로는 민감도가 59%, 특이도가 73%로 낮아서 위암 환자를 선별할 수 없으며[58], 제균 치료 후에는 ABC 분류가 정상화되는 과거 감염자들이 많아서 고위험군을 감별하기가 더욱 어려워진다[59]. 예를 들면, 조기위암을 내시경 점막하 박리술로 절제하고 제균 치료한 뒤에 추적 검사를 받은 환자들은 과거 감염자임에도 불구하고 대부분이 ABC 분류에서 A군으로 분류된다[60]. 즉, 혈청학적 위축이 없는 과거 감염자는 ABC 분류상 정상으로 보고되므로 위암 선별 검사로 부적합하다[61].

3. 혈청 PG 검사의 한계점

1) 과거 감염자에서의 위음성 소견

과거 감염자는 미감염자에 비해 위암 발생률이 높지만[62], 혈청검사나 위내시경 소견은 정상 소견으로 보일 수 있다[63]. 본원에서 PG 수치가 정상으로 보고된 H. pylori 혈청항체 음성자 2,620명을 분석해보니 22.2%는 과거 감염자로, 다른 치료를 받으면서 의도치 않게 우연히 제균되거나 균의 자가소멸 후에 위점막이 회복된 경우도 있었다[64]. 이들은 ABC 분류 시 D군처럼 미감염자에 비해 장형 위암이 발생할 위험성이 높음에도 불구하고 A군으로 잘못 분류된다(Table 3).

Limitations and Possibilities of ABC Classification

일본의 다기관 연구에 의하면 PG I ≤31.2 ng/mL와 PG I/II ≤4.6인 경우는 과거 감염자일 가능성이 높았으며[65], 다른 연구에서는 PG I ≤37.0 ng/mL와 PG I/II ≤5.1이면 과거 감염자일 가능성이 높았다[66]. 이들은 본인도 모르는 사이에 감염되었다가 치료된 경우도 있어 병력 청취로 미감염자와 구별할 수 없으므로, 화생성 위염이나 황색종 또는 체부까지 진행된 위축성 위염이 내시경으로 관찰되면 과거 감염자로 분류하는 것이 바람직하다. 혈청 검사에서 정상 소견을 보여도 폐쇄형 위축성 위염이 있으면 0.07%에서 위암이 함께 진단되고, 개방형 위축성 위염이 보이면 0.8%에서 위암이 진단된다는 보고도 있다[67]. 한국인에서 체부의 위축성 위염과 연관된 수치는 PG I <59.3 µg/L (민감도 83.3%, 특이도 78.4%)과 PG I/II 비 <3.5 (민감도 70.0%, 특이도 78.4%)로[68], 전정부의 위축성 위염에 비해 혈청 PG 수치와 유의한 연관성을 보인다[69]. 전정부의 위축성 위염과 연관된 수치는 PG I/II 비 <4.9로 범위가 넓어서 임상적 유용성이 낮다(민감도 68.2%, 특이도 60.3%) [70].

2) 신부전과 약물복용으로 인한 PG 수치의 증가

만성 신부전 환자들은 PG가 소변으로 배출되지 않으므로 수치가 높게 나와서 PG I/II 비 ≤3이 아닌 ≤7.75로 H. pylori 감염자를 선별한다[71]. 또한, 아스피린 등 위점막을 자극하는 약물이나 장기간 벽세포를 억제하는 위산억제제를 복용하는 경우에는 PG I 분비가 항진되므로, PG II보다 PG I 수치의 상승이 두드러진다면 H. pylori 감염보다 약물로 인한 위점막 손상을 의심해야 한다.72 이를 고려하지 않으면 혈청학적 위축이 있는 신부전 환자나 약제성 위염이 있는 D군을 A군으로 오진하거나, C군을 B군으로 오진할 수 있다(Table 3).

3) 위절제술 후의 PG 수치의 감소

위암으로 인한 위절제술 외에도 궤양 천공이나 비만 치료를 위한 수술 후에도 PG 수치가 감소할 수 있다[73]. 따라서 위절제술 후에 PG II 수치가 감소해도 균 소멸로 판단하면 안 되며, 다른 H. pylori 검사로 감염 여부를 확인해야 한다. 기저부나 체부를 절제한 A군이나 B군은 PG I 수치가 낮게 측정되므로, 각각 D군이나 C군으로 잘못 분류될 수 있다(Table 3).

4) 인종이나 지역에 따른 차이

혈청 PG 검사는 위축성 위염에서 발생하는 비분문부 암을 선별하기 위한 검사로, 위식도 접합부나 분문부의 암은 선별하지 못한다[74]. 따라서 H. pylori 균에 의한 비분문부 암이 드문 서양에서는 위암검진용 PG 검사가 비효율적이다[75]. 다른 백인들에 비해 상대적으로 위암 발생률이 높은 일부 지역에서만 PG 검사가 유용하며[76], 위암 진단에 도움이 되는 PG 수치도 지역마다 다르다(Table 4) [24,77-87]. H. pylori 감염자에서 위암이 많이 발생하는 한국과 중국에서는 높은 PG II 수치가 위암 환자 선별에 유용하지만[24,81], 2013년부터 전 감염자에 대한 제균 치료를 해서 과거 감염자에서 발생하는 위암이 더 많아진 일본에서는 낮은 PG I/II 비가 유용하다[78-80]. 전 감염자에 대한 제균 치료를 시작한 2013년부터 PG II 수치가 높은 미만형 위암 환자가 거의 사라졌고, 미만형 위암으로 진단되더라도 대부분이 장형 위암에서 진행한 경우라서 PG II 수치가 낮기 때문이다. 그 외 다른 국가에서는 낮은 PG I 수치와 PG I/II 비를 종합해야 위암 환자 선별에 도움이 된다[85-87]. 즉, ABC 분류를 효과적으로 사용하기 위해서는 우리나라나 중국처럼 현감염자가 많은 나라에서는 B군과 C군에서 발생하는 미만형 위암을 놓치지 않도록 주의하고, 일본처럼 과거 감염자가 많은 나라에서는 D군과 가성 A군(pseudo group A)에서 발생하는 장형 위암에 주의해야 한다. 한편, 미감염자가 많은 서양에서는 D군에 섞여 있는 자가면역성 위염 환자들을 놓치지 않도록 주의해야 한다(Table 3). 자가면역성 위염은 H+/K+-adenosine triphosphatase를 항원으로 인식하는 항벽세포 항체가 위저선의 벽세포와 주세포를 파괴하는 질환으로, PG I 분비가 감소하여 D군으로 분류되기 쉽기 때문이다[88].

결 론

지금까지 혈청 PG 검사는 낮은 PG I 수치와 PG I/II 비로 위축성 위염이 심한 장형 위암의 고위험군을 감별하기 위한 선별 검사로 주로 사용되었으나, 우리나라처럼 미만형 위암이 흔한 지역에서는 높은 PG II 수치도 중요하다. PG II 수치가 높으면서 PG I/II 비가 낮을 때 H. pylori 감염자일 가능성이 높다는 것을 알아야 침습적인 검사의 위음성 소견을 간과하지 않기 때문이다.

대부분의 감염자들은 제균 성공 시 수개월 안에 H. pylori에 대한 혈청 IgG 수치가 50% 이상 감소하고, PG II 수치가 25% 이상 감소하며, 1년 안에 PG I/II 비가 5.0 이상으로 증가한다. 하지만 위점막의 손상 정도에 따라 변화폭과 기간이 다르고 위절제술, 신부전, 복용약물에 따라 PG 수치가 변하며, PG II 수치의 변화는 미만형 위암을 유발하는 H. pylori 감염 시에만 관찰되는 등 변수가 많다. 아울러 균이 사라진 뒤 10년이 지나도 혈청음전이 되지 않거나 위점막의 손상이 심해서 혈청학적 위축이 지속되는 경우도 있으므로, 혈청 검사는 제균 확진용으로 사용할 수 없다. 또한, 제균 실패 시에는 바로 다른 항생제로 치료해야 하므로, 제균 여부에 대한 판단은 혈청 수치가 감소할 때까지 기다리지 않고 항생제 복용을 마친 후 4주가 지난 시점에서 13C-UBT나 단클론성 대변항원 검사로 확인해야 한다. 새로 H. pylori에 감염되어도 혈청 IgG 수치로 반영되기까지 수개월이 필요하다는 점을 감안할 때, 혈청 검사는 확진용 검사가 아니라 수개월에서 수년간의 변화를 위내시경 소견과 함께 맥락적으로 해석해야 하는 검진용 검사로 취급되어야 한다.
