Trans Abstract

Barrett’s esophagus (BE) is caused by metaplasia of squamous epithelium into columnar epithelium, mainly in the lower esophagus. Updates have been made in the etiology, diagnosis and treatment of BE since its first report as an ulcerative lesion in the lower esophagus. Columnar metaplasia of the lower esophagus has been reported as a result of gastroesophageal reflux, rather than genetic factors. When specialized intestinal metaplasia (SIM) is confirmed on biopsy, it is diagnosed as BE; otherwise, it is described as columnar line esophagus (CLE). Recent recommendations state that the following two conditions must be satisfied to diagnose BE: CLE with a length ≥1 cm proximal to the gastroesophageal junction, or SIM, with goblet cells confirmed on biopsy. BE is a key risk factor for esophageal adenocarcinoma. Early detection and treatment of BE, dysplasia, and early esophageal adenocarcinoma arising from BE has become a critical issue in the USA and Europe. The prevalence of BE in Korea is likely to increase because of the increasing prevalence of gastroesophageal reflux diseases. However, there are few reports on the diagnosis and treatment of BE in Korea. In this narrative review, important historical discoveries related to BE, the anatomical structures and endoscopic findings around the gastroesophageal junction required for diagnosing CLE and BE, and a brief review of the guidelines for the diagnosis and treatment of BE are summarized.

서 론

1950년경 영국의 흉부외과 의사인 Norman Barrett은 식도에 발생한 원주상피로 둘러싸인 하부식도 궤양 증례를 보고하면서, 하부식도의 편평상피가 원주상피로 대체된 병리 소견에 대하여 기술하였다. 이 즈음부터 원주상피로 대체된 식도(columnar lined esophagus, CLE)의 개념과 병인에 대한 중요성이 부각되기 시작하였다[1,2].

많은 연구에 근거하여 현재에는 바렛식도는 식도 선암종의 전암성 병변으로 잘 알려져 있으며[3] 서구에서 식도 선암종은 증가하는 추세에 있고[4], 미국에서는 식도암의 60% 정도가 선암종으로 보고되고 있다[5]. 이런 사유로 식도 선암종으로 인한 사망률을 감소시키기 위하여 바렛식도 및 바렛식도에서 기원한 이형성증과 조기 식도 선암의 진단과 치료에 대한 관심이 증가하고 있다[6].

그러나 한국에서는 전체 식도암 중에서 편평상피세포암이 식도암의 90%, 식도 선암은 9%의 비율로 보고되고 있고, 편평상피세포암과 식도 선암종의 발생률이 모두 과거에 비하여 감소추세에 있다 [7]. 그리고 우리나라에서 바렛식도의 정확한 유병률이나 바렛식도에서 기원한 식도 선암종의 발생 위험을 잘 규명한 연구나 발생률을 조사한 연구가 없는 실정이기에 단언하여 언급할 수는 없으나, 국내의 바렛식도의 유병률은 1% 내외로 매우 낮을 것으로 추정된다 [8,9]. 그러므로 한국의 의사들은 바렛식도에 대한 관심이 적고, 바렛식도의 진단과 치료의 필요성에 대한 임상적인 주의는 거의 없는 것이 현실이다.

이러한 상황에서 ‘국내에서 단분절 바렛식도에서 이형성이 없는 경우 환자에게 식도암의 위험성을 크게 강조하여 설명할 필요는 없을 것으로 판단되며 바렛식도 환자의 약물 요법과 이형성을 가진 바렛식도 환자에서 서구의 가이드라인을 따르는 것이 타당할 것’으로 기술하였던 바렛식도에 대한 이전의 리뷰의 제언[10]은 여전히 유효한 조언으로 판단된다.

따라서 아직까지는 서구의 권고안에 근거하여 바렛식도의 진단에 대한 개념과 제안된 치료 방안에 대한 이해를 통해 우리나라에서는 이를 어떻게 적용할 것인가에 대한 접근이 필요할 것으로 판단된다.

이 글에서는 바렛식도와 연관되었던 역사 속의 논란들에 대하여 살펴보고, 바렛식도의 진단에 있어서 내시경 소견 및 병리 소견에 대한 이해, 바렛식도의 치료 및 추적 관찰에 대한 가장 최근 권고안의 제안 사항에 대하여 살펴보고자 한다.

본 론

1. 역사 속의 바렛식도의 역사와 논쟁점

Norman Barrett이 식도에 발생한 소화성 궤양을 최초로 보고한 것은 아니지만 식도에 발생한 원주상피로 둘러싸인 소화성 궤양을 보고한 바 있고[1], 이후 1960년도에 이르기까지 발표한 저술에서 식도염의 결과로 편평상피가 원주상피로 대체될 수도 있으며, 원주상피로 화생(metaplasia)된 식도가 식도 선암의 전구 병변일 수 있음을 기술하기도 하였다[2]. 그러나 Norman Barrett은 역류성 식도염이 반복되고 식도가 짧아져 위의 분문부를 흉강으로 끌어 올려 횡격막(diaphragm) 위에 원주상피가 존재하는 것으로 기술한 바 있고, 식도 궤양의 병인(etiology)에 대한 의견을 제시하였던 초기에는 식도 하부가 원주상피 세포로 대체되어 있기 때문에 식도염이 유발되는 것으로 판단하였다[11]. 또한, 역류성 식도염의 결과로서 편평상피가 원주상피로 화생된다는 이론을 지지하지 않았으며, 초기의 저술에서 식도의 원주상피가 존재하는 원인 역시 원주상피로 화생된다는 이론보다는 식도의 수축으로 위의 분문부의 일부가 횡격막의 상부인 흉강으로 이동한 것으로 이해하였던 것으로 추정된다. 그러므로 Norman Barrett은 아마도 식도 하부의 궤양은 식도의 궤양이라기보다는 위의 분문부에 발생한 궤양으로 생각하였던 것으로 보인다[1].

실제로 식도에 발생한 소화성 궤양과 식도에 존재하는 원주상피에 대한 보고는 그보다 앞선 1906년에 보고된 바 있고[12] 역류성 식도염(reflux esophagitis)의 개념과 원주상피로 대체된 식도의 개념 역시 Allison [13,14]에 의하여 Norma Barrett보다 앞서 기술되었다. 현재까지 받아들여지고 있는 바렛식도의 병인으로 잘 알려진, 편평상피세포로 구성된 하부식도가 위식도 역류에 의하여 원주상피로의 화생된 결과물이라는 추측에 대한 돌파구를 제시한 것 역시 Norman Barrett이 아닌 Hayward [15]와 Adler [16]였다.

이러한 사실들에 근거하여 Norman Barrett이 식도 궤양 및 주변부에 존재하는 원주상피세포에 대한 최초 보고자가 아니며, 바렛식도의 병인과 관련된 핵심적인 개념에 대한 기여한 바가 없고, 원주상피로 대체된 식도의 내시경 소견에 대하여 기술한 바가 전혀 없으므로 ‘바렛식도’라는 명명법(eponyms) 자체에 대한 반대 의견이 있고, 원주상피로 대체된 식도라는 용어를 사용하여야 한다는 의견이 있다[17]. 그러나 이러한 논란에도 불구하고 현재까지는 바렛식도라는 용어가 널리 받아들여지고 있다.

바렛식도와 관련된 연구는 1950년도에는 진단 기준과 병인론에 대한 보고들이 많았으며, 1960년도에는 바렛식도와 식도 선암종의 연관성에 대하여 밝히고자 하였다. 1970년부터 1990년도까지는 전암성 병변으로서 바렛식도의 감시에 초점을 두었으며, 1990년도 이후에는 바렛식도에 대한 새로운 내시경적 기술들을 적용하고자 하는 노력들이 주를 이루고 있다[18,19].

2. 병인론

최근에는 위식도 역류에 의한 손상으로 인한 식도의 편평상피의 원주피로의 화생이라는 가설이 가장 널리 받아들여지고 있으나 1950년도에는 이것을 선천적(congenital) 질환으로 이해하려는 시도가 있었다. 바렛식도는 위식도 역류에 의한 손상으로 인한 결과로 나타나는 것임을 밝혀내는 데 중요한 단서를 제공한 Hayward는 그의 논문에서 위식도 접합부(gastro-esophageal junction)의 주변부의 구조에 대한 개념을 제시하였는데, 위식도 접합부의 분문부에는 편평상피세포와 원주상피세포가 동시에 존재할 수 있고 위식도 접합부의 상피(junctional epithelium)는 위나 식도 양측으로 확장될 수 있을 것으로 추측하였다(Fig. 1) [15].

이후 Adler는 원주상피로 대체된 식도 상피의 후천적인 변화의 원인으로서 다음과 같은 3가지 가능성을 제시하였다: 1) 흉강 내로 위의 이동(upward extension from the stomach), 2) 편평상피세포의 선상피로의 화생(metaplasia from squamous epithelium), 3) 식도 상피 표층에 존재하고 있던 원주상피(from the superficial cardiac glands of the esophagus). 그의 보고에서 250명의 부검 식도 조직 표본 검사 결과를 토대로 하여 원주상피로 대체된 식도의 경우, 종종 편평상피가 벗겨진 식도를 덮기 위해 원주상피가 자라는 것처럼 관찰되었고, 식도의 더 깊은 층을 외부 손상으로부터 보호하기 위하여 원주상피의 과형성(hyperplasia)을 보이며, 점액 분비를 증가시키는 것처럼 관찰됨을 기술하였다[16]. 이러한 원주상피로 대체된 식도상피는 인생의 후반부에 발생하며 식도의 궤양이나 협착의 결과로 인한 경우가 흔한 것으로 보고하였는데, 이는 결과적으로 '편평 상피세포의 선상피로의 화생'이라는 2번째 가설을 뒷받침하는 증거로 판단된다.

3. 조기 식도 선암과의 연관성에 대한 보고

1950년도에 상부 1/3 (upper one third) 식도에 위치한 이소성 위점막에서 발생한 조기 식도 선암이 최초로 보고되었고[20], 바렛식도와 조기 식도 선암에 연관성에 대한 보고가 이어졌다[21]. 이러한 조기 식도 선암의 조직 표본에서 병변의 근위부에서부터 원위부에 이르기까지 원주상피가 존재함을 확인하면서 바렛식도와 조기 식도 선암과 연관성에 대한 증거는 더욱 확실해졌다[22]. 1955년, 1956년에 발표된 연구에서 식도의 원주상피에서 기원한 조기 식도 선암이 전체 식도암의 8~10%에서 관찰됨을 보고하였다[23,24]. 2018년도의 미국의 보고에서는 조기 식도암의 60%가 식도 선암으로 보고되었는데, 위식도 역류 질환의 증가 및 바렛식도 유병률의 증가로 기인하였을 가능성이 있다. 1990년도의 보고에서는 바렛식도에서의 식도 선암의 발생률을 208인 년당 1명[25] 285인 년당 1명으로 보고한 바 있다[26].

4. 단분절 바렛의 개념의 등장

1960년도까지는 하부식도의 1∼2 cm 정도는 정상적으로 원주상피로 구성될 수 있으며 식도와 위 사이의 완충 영역으로 작용한다고 믿어져 왔다[15]. 따라서 1∼2 cm 정도 짧은 길이의 원주상피로 대체된 식도는 바렛식도의 진단 기준에서 제외되었고, 하부식도에서 원주상피의 길이가 3 cm 이상인 경우에만 바렛식도로 진단하도록 제안되어 왔다. 그러나 1994년도 Spechler 등[27]은 3 cm보다 짧은 길이의 하부식도 원주상피를 보인 연구 대상자 중 18%에 해당하는 대상자의 생검 조직에서 특수 장상피화생이 관찰됨을 보고하였다. 그리고 식도 선암의 증가 요인으로, 단분절 바렛식도에 대한 진단이 늦어지기 때문인 것으로 추정하였다[27]. 이후 3 cm보다 짧은 길이의 원주상피로 대체된 식도에서도 전암성 변화로 인정되는 장상피화생이 높은 빈도로 발견되었기에, 3 cm를 초과하는 바렛식도를 장분절 바렛식도(long segment Barrett esophagus), 3 cm 이하의 바렛식도를 단분절 바렛식도(short segment Barrett esophagus)로 구분하여 기술하게 되었다.

5. 바렛 식도의 정의

가장 최근의 미국 소화기학회와 유럽 소화기내시경학회의 정의에 의하면 바렛식도는 식도 하부 점막의 편평상피가 위점막을 구성하는 원주상피로 화생 변형된 상태를 말한다[28,29]. 조직학적으로는 배세포를 동반한 특수 장상피 화생(specialized intestinal metaplasia, SIM)이 동반되어야 하며, 위식도 접합부에서 근위부로 적어도 1 cm 이상 돌출된 경우로 정의하였으며, 1 cm 이하의 경우 조직 검사를 시행하지 않을 것을 권고하고 있다[28,29]. 장상피화생이 없는 원주상피로 대체된 식도의 경우에는 식도 선암의 위험성이 증가하지 않는다고 보고되고 있기 때문이다[30]. 배세포를 동반하지 않은 경우에도 바렛식도를 진단할 수 있다고 2014년 영국 소화기학회의 권고안[31]에 제안되었으나, 아직까지는 널리 받아들여지고 있지는 않다. 이후의 내용에서 독자들의 오해를 피하고자 원주상피로 대체된 식도를 서술한 경우 배세포를 동반한 특수 장상피화생(SIM)이 동반되지 않은 상태로 이해하기를 바란다.

6. 진단

1) 위식도 접합부의 해부학적 구조에 대한 이해와 내시경 소견

내시경적으로 바렛식도를 의심하고 진단하기 위하여는 위식도 접합부의 해부학적 구조에 대한 이해와 내시경 소견에 대한 이해가 선행되어야 한다. 일반적으로 정상인에서는 위식도 접합부와 편평상피-원주상피 접합부(squamo-columnar junction)가 일치하며, Z-line으로도 불리우는 편평상피-원주상피 접합부가 하부식도 괄약근(lower esophageal sphincter)의 가장 아래 부분에 위치하면서, 횡격막 식도 열공보다 약간 아래쪽에 위치하므로 내시경이 식도에서 위로 진입하면서 관찰하는 경우에 Z-line을 정확하게 확인할 수 없는 경우가 많다(Fig. 2A). 내시경 소견을 이용하여 바렛식도를 관찰하고 진단을 계획하는 경우 조직 검사를 시행하기 전에 바렛식도를 의심하여야 하는 소견은 위식도 접합부보다 상방으로 이동한 편평상피-원주상피 접합부(또는 Z-line)가 확인되는 경우다(Fig. 2B). 편평상피-원주상피 접합부는 육안 소견으로 경계가 비교적 뚜렷하게 지어지는 선(Fig. 2C)이므로 인지하기가 쉬우나 위식도 접합부의 경우 위치를 정확하게 알기는 어렵다. 다만 일반적으로 위 주름의 근위부가 위식도 접합부의 판정의 일차 기준이며(Fig. 2D), 하부식도의 울타리 영역(palisade zone)의 원위부가 이차적인 기준이 된다(Fig. 2E) [29,32]. 위식도 접합부 판정은 내시경을 통한 관찰자나 기준에 따라 다소 차이가 발생할 가능성이 있으며, 많은 사람에서 위식도 접합부와 편평상피-원주상피 접합부(또는 Z-line)가 완벽하게 일치하지 않을 수 있다(Fig. 2F).

그러므로 위식도 접합부와 편평상피-원주상피 접합부의 위치 관계를 정확하게 파악하면 바렛식도의 진단에 도움이 된다. 위 주름의 근위부를 위식도 접합부로 정의한 경우 원주상피로 대체된 식도는 노란색으로 강조 표시한 영역이 되며(Fig. 3A, B), 하부식도의 울타리 영역의 원위부를 위식도 접합부로 정의한 경우에는 원주상피로 대체된 식도의 범위가 달라진다(Fig. 3C, D). 바렛식도의 진단과 치료와 관련한 거의 대부분의 권고안에서 위 주름의 근위부를 위식도 접합부로 정의하고 있으나 일본 식도학회에서는 하부식도의 울타리영역의 원위부를 위식도 접합부로 정의하고 있다[33,34].

바렛식도를 의심해야 하는 가장 중요한 내시경 소견은 위식도 접합부보다 근위부에서 관찰되는 Z-line을 확인하는 것이다. 다만 가장 최근의 바렛식도의 진단 기준을 만족하기 위하여는 원주상피로 대체된 식도의 최소 길이가 1 cm 이상인 경우이므로 Fig. 3B, D의 붉은색 화살표로 표시된 원주상피로 대체된 식도 점막의 길이를 확인하고 이후 조직 검사를 통해 최종 진단하는 것이 좋다. 원주상피로 대체된 식도는 위점막과 비슷한 연어빛 원주상피인데, 근접하여 관찰하면 융모 같은 패턴이 관찰되거나(Fig. 4A), 상피하 혈관이 관찰되기도 한다. 그러나 식도염이 있거나 협착이 있는 경우에는 위와 같은 소견이 잘 관찰되지 않을 수 있다.

바렛식도에서 편평상피-원주상피 접합부는 매우 다양한 모양을 보일 수 있는데, 위식도 접합부의 상방에 위치하는 매끈한 직선 혹은 부드럽게 물결치는 곡선으로 관찰되는 경우가 많지만, 손가락 모양으로 상방으로 펼쳐진 원주상피가 동반된 예도 많다. 간혹 편평상피-원주상피 접합부 근위부에 정상 식도도 둘러싸인 원주상피 섬(columnar island)이 보일 수 있다. 많은 수의 바렛식도, 특히 길이가 긴 바렛식도는 대부분 열공허니아를 동반하고 있다(Fig. 4B).

2) 백색광 내시경 외 내시경 진단 기법

색소 내시경을 이용한 바렛식도의 진단에는 루골용액(lugol solution)이나 메틸렌블루를 이용할 수 있다. 루골용액은 Z-line을 확인하는 데 도움이 되며 메틸렌블루의 경우 바렛식도의 원주상피로 대체된 식도에 염색이 되는데 주변과 염색 정도가 다른 경우 이형성이나 악성 변화를 동반하는 경우가 있다[35].

협대역 내시경(narrow band imaging)과 자가형광 내시경(autofluorescence endoscopy) 등이 바렛식도 및 이형성의 진단에 유용하다는 보고가 있으나[36,37], 협대역 내시경의 경우 인디고카민을 이용한 색소 확대 내시경 및 확대 내시경 단독 사용과 비교하였을 때 바렛식도 및 동반된 이형성증이나 식도 선암의 진단의 민감도에 차이가 없었다고 보고되었고[38], 자가형광 내시경의 경우 조직 검사와 비교하여 진단의 민감도가 낮아 조직 검사를 대체할 수 없다고 보고되었다[39]. 그러므로 아직까지는 백색광 내시경과 비교하여 협대역 내시경이나 자가형광 내시경이 향상된 진단 능력을 증명하였다고 판단되지는 않으며, 조직 검사를 대체할 수 있을 만한 기술적 발전을 이루었다고 판단되지 않는다.

3) 바렛식도의 길이 측정과 Prague 기술법

식도 관강의 360도를 둘러싸 원주상피로 화생된 점막과 이로부터 좀 더 근위부의 식도까지 혀 모양으로 관강의 일부만이 원주상피로 화생된 경우 Prague C-M 방식으로 길이를 기술할 수 있다. 관강의 원주형으로 관찰되는 범위까지의 최대 길이(C)를 표시하고 관강의 일부에서 혀 모양으로 식도의 근위부로 확장된(tongue-like projection)을 포함한 병변의 최대 길이(M)를 표시하는 방식이다(Fig. 5) [40].

4) 바렛식도의 조직 검사 방법

조직 검사의 목적은 특수 장상피화생을 증명하여 병리학적으로 진단하는 것과 고도 이형성이나 식도암을 조기에 발견하여 조기에 치료하는 데에 있다. 고전적으로 시애틀 프로토콜이라고도 부르는 조직 검사법이 추천되어 왔다. 이 방법은 위식도 접합부에서부터 편평상피-원주상피 접합부까지 각 2 cm 간격으로 네 방향에서 하나씩 조직을 채취하는 것을 말한다[41,42].

가장 최근의 가이드라인 모두 시애틀 프로토콜에 따라 조직 검사를 시행하도록 권고하고 있으며, 특정 부위의 점막 이상 소견이 관찰되는 경우 추가적으로 점막 이상을 보이는 부위에서도 조직을 채취할 것을 권고하였다[28,29].

경험이 많은 전문가가 바렛식도에서 시애틀 프로토콜에 따라서 조직 검사를 시행한다고 하더라도 천공이나 출혈과 같은 합병증은 발생하지 않았고 대부분의 합병증들이 회복될 수 있었다고 보고하여 안전한 조직 검사 방안임을 보고하였다[43]. 그러나 시애틀 프로토콜대로 조직 검사를 시행하더라도 위음성이 나올 가능성이 있고 많은 시간이 소요되는 등의 문제가 제기되고 있다[44,45]. 최근에는 아세트산(acetic acid)을 이용한 표적 생검(target biopsy)의 효용성들이 연구 및 보고되고 있다[46].

현재 국내에서 바렛식도의 조직 검사에 대한 연구나 권고안은 없다. 게다가 국내 소화기내과 전문의를 대상으로 한 조사에서 시애틀 프로토콜에 따라 조직 검사를 시행하겠다고 응답한 비율은 9.9% (91명 중 9명)였다[47]. 우리나라에서는 바렛식도의 유병률이 매우 낮고[48], 전체 식도암 중 식도 선암의 비율은 낮으며[7] 바렛식도에서 병발한 조기 식도 선암에 대한 증례보고의 대부분이 하부식도 및 위식도 접합부에 발생한 경우가 많고, 대부분 백색광 내시경으로 정상 조직과 구분되는 점막의 이상 소견을 보이는 경우가 대부분이었다[49-52]. 우리나라에서도 동일하게 시애틀 프로토콜을 적용하여야 하는가에 대하여는 추가적인 연구를 통해 밝혀져야 하겠으나, 실제 임상 현장에서 시애틀 프로토콜을 시행하지 않고 있으며, 국내 대다수의 전문가들은 표적 생검을 권고하고 있는 것이 현실이다.

5) 바렛식도의 조직학적 소견

앞에서 언급한 것과 같이 원주상피로 대체된 식도라는 정의는 편평상피가 원주상피로 화생되었다는 조직학적 개념을 포함한다. 이러한 원주세포(columnar cell)의 존재만으로는 바렛식도를 진단할 수 없다. 원주상피로 대체된 식도의 상피에서 시행한 조직 검사 소견에서 배세포(goblet cell)가 intermediate mucous cell 사이에서 관찰되어야 하며 이러한 소견은 상피의 표층(surface) 및 선상피(glandular epithelium) 모두에서 관찰되어야 한다[53]. 이러한 특수 장상피화생(SIM)을 동반할 때 전암병변으로서 바렛식도를 진단할 수 있다. 바렛식도의 조직학 소견은 크게 이형성성 없음(negative for dysplasia), 불명(indefinite), 이형성(positive for dysplasia), 점막암(intramucosal carcinoma)의 4개 분류로 구분하며, 이형성의 경우 저등급 및 고등급으로 추가하여 기술할 수 있다[53].

바렛식도에서 이러한 특수 장상피화생은 CK7 및 CK20을 이용한 면역 형광 염색을 이용하면 조직학적 진단에 도움을 받을 수 있다[54]. 또한 MUC2 유전자 염색을 하면 특수 장상피화생을 보이는 바렛식도에서 배세포에 염색되어 관찰되는데, 식도 선암이 발생하게 되면 MUC2 유전자의 발현이 줄어들고 MUC1 유전자가 발현되면서 식도 선암 세포에 MUC1 유전자 염색되어 관찰됨을 보고하기도 하였다[55].

7. 바렛식도의 추적 관리 및 치료

바렛식도의 선별 검사의 필요성과 추적 내시경 검사 및 이형성증이나 조기 식도 선암이 발생한 경우의 치료와 관련하여서는 국내의 권고안이 없으므로 미국 소화기학회 및 유럽 소화기내시경학회의 권고안을 중심으로 기술하였다.

1) 선별 검사 및 진단을 위한 조직 검사

미국 소화기학회 및 유럽 소화기내시경학회 바렛식도 임상진료 지침 권고안 모두에서 전체 인구 집단에서 바렛식도의 내시경 선별 검사를 권고하지는 않고 있다. 다만, 장기간의 위식도 역류 증상과 복수의 기타 위험인자(고령, 백인, 남성, 비만, 바렛식도나 식도 선암의 가족력)를 가지고 있는 고위험군의 대상자에서 선별 검사가 필요하다고 기술하였다. 유럽과 미국 모두에서 조직 검사는 시애틀 프로토콜에 따른 조직 검사를 권고하였다.

2) 추적 내시경 검사

이형성증을 동반하지 않은 바렛식도의 경우 검사의 이득과 위험도를 고려하여 추적 내시경 검사를 고려해 볼 수 있다고 기술하였으며[28], 유럽 소화기내시경학회 권고안에는 바렛식도의 길이에 따라 1~3 cm의 경우 5년, 3~10 cm의 경우 3년으로 추적 내시경의 구체적인 추적 간격을 제안하였고[29], 영국의 소화기학회의 권고안에서는 3 cm 미만인 경우 3~5년 간격으로, 3 cm 이상인 경우 2~3년의 추적 관찰을 제안하였다(Table 1) [56].

3) 불명확한(indefinite) 이형성증 및 저등급 이형성증

바렛식도의 조직학적 진단을 위해서는 모든 권고안에서 소화기병리 전문가의 확진이 필요함을 언급하였고 불명확한(indefinite) 이형성증을 보이는 경우에는 먼저 산분비 억제제를 3~6개월간 사용한 뒤에 추적 내시경 검사 및 조직 검사를 재시행할 것을 권고하였다. 저등급 이형성증으로 확진된 경우에도 하루 두 차례의 양성자펌프억제제를 8~12주간 투약한 후 고해상도 백색광 내시경으로 반복 검사를 시행할 것을 권고하였다[57]. 점막의 이상을 보이고(nodular Barrett's esophagus or visible abnormality) 저등급 이형성증을 동반한 바렛식도의 병변에 대하여는 내시경 절제(endoscopic resection)를 권고하고 있으나, 영국의 권고안에서는 절제를 강하게 권고하지는 않았다. 점막의 이상이 없는(non-nodular or invisible abnormality) 저등급 이형성증을 동반한 바렛식도는 고주파 치료와 같은 내시경근치술(endoscopic eradication)을 권고하였으나, 영국의 권고안에서는 모든 사례에서 내시경 근치술이 권고되지는 않는다고 기술하였다(Table 2).

4) 고등급 이형성증과 조기 식도 선암

육안상 점막 이상 소견이 있고 고등급 이형성증이 진단된 경우에는 해당 병변에 대하여 내시경 절제술을 시행하도록 권고하였다. 육안상 점막의 이상 소견이 관찰되지 않으나 조직 검사에서 고등급 이형성이 확인되면 미국 소화기학회에서는 내시경 근치술을 권고하였고, 유럽 소화기학회에서는 조직 검사를 다시 시행하여 이형성증이 확인되지 않으면 3개월 후 재검을 한 뒤에 여전히 고등급 이형성이 보고되는 경우에 내시경 근치술을 시행하는 것을 권고하였으며, 영국 소화기학회에서는 고해상도 내시경(high resolution endoscopy)을 시행하여 점막의 이상 소견을 특정한 후에 내시경 절제술을 먼저 고려하고, 점막의 이상이 발견되지 않으면 내시경 근치술을 시행하는 것으로 기술하였다[56].

점막층에 국한된 T1a 병기 조기 식도 선암의 경우에는 미국과 유럽 권고안 모두 내시경 절제술을 권고하고 있고, 점막하층을 침윤한 T1b 병기의 조기 식도 선암의 경우에는 조건에 따라 다학제 진료를 시행하고 내시경 절제술을 고려할 수 있다고 제안하였다[28,29,56]. 고등급 이형성증이나 조기 식도 선암이 발견된 경우 남아있는 바렛식도는 고주파 열치료와 같은 내시경 근치술을 시행하도록 권고하였다(Table 3).

5) 양성자펌프억제제의 예방적 투약

위식도 역류 질환으로 인한 위산의 역류가 바렛식도의 병인으로 알려져 있다. 실제로 산분비 억제제를 통해 바렛식도의 진행을 예방하고 바렛식도에서 기원하는 이형성증이나 식도 선암의 발생을 감소시킬 수 있다는 연구 결과들이 보고되고 있다. 이형성증이 동반되지 않은 바렛식도에서 양성자펌프억제제의 투약의 예방 효과에 대한 메타분석에서 양성자펌프억제제의 투약이 고등급 이형성증 및 식도 선암의 발생 위험도를 낮추는 것으로 보고하였다[58,59]. 다만 환자 대조군 연구만 분석한 경우 예방 효과가 통계적으로 유의미하지 않은 결과를 제시하기도 하였다[60]. 그러므로 바렛식도의 합병증으로서 전암성 병변 및 식도 선암과 같은 질환의 예방을 위해 양성자펌프억제제의 사용은 중요한 의학적 이슈가 되고 있다.

미국 소화기학회에서는 바렛식도 환자에서 하루 1번의 투약을 권고(strong recommendation, moderate level of evidence)하였다. 또한 위산 역류가 조절되지 않는 경유를 제외하고는 이유 없이 하루 2회 투약을 하지 않도록 주의할 것을 기술하였다[28]. 다만 영국의 소화기학회의 권고안에서는 바렛식도 환자에서 위산으로 인한 증상이 있는 경우에는 증상 조절을 위해서 양성자펌프억제제의 사용은 권고하였으나, 화학예방요법(chemopreventive agent)으로서 전암성 병변 및 식도 선암과 같은 질환을 예방한다는 증거는 다소 부족하다고 판단하였다(recommendation grade C).

그러므로 적어도 우리나라에서도 조직학적으로 진단된 바렛식도 환자가 위산 역류의 증상이 동반되어 있는 경우에 양성자펌프억제제 투약을 권고하는 것이 필요하다고 판단된다.

결 론

바렛식도의 진단을 위하여 원주상피로 대체된 식도의 내시경 진단과 배세포를 동반한 특수 장상피화생의 조직학적 소견의 필요성 그리고 바렛식도에서 기원하는 이형성증이나 조기 식도 선암과의 치료에 대하여 간략히 알아보았다. 바렛식도의 명명에 대한 반론도 있으나 식도 선암의 전암 병변으로서 앞에서 언급한 개념을 모두 포함하여 간략하게 제시할 더 나은 방안이 없는 지금 그리고 가까운 미래에도 해당하는 명칭과 개념은 유지될 것으로 판단된다.

서구에서는 바렛식도의 유병률이 높아 비교적 흔하게 접하게 되므로 해당 질환의 연구가 활발하게 이루어지고 있으나, 바렛식도의 유병률이 낮은 국내에서 바렛식도 및 합병증과 관련된 국내의 연구 결과는 많지 않은 것이 현실이다. 다만 아시아에서는 일본에서의 바렛식도의 유병률이 타 아시아 지역에 비하여 높은 것으로 분석되기도 하였는데[61], 이러한 차이가 진단 기준의 차이 외에도 원주상피로 대체된 식도의 개념과 내시경 소견에 대한 인식과 조직 검사를 시행하였는지 여부에 영향을 받았을 가능성도 있다고 판단된다. 실제로 우리나라의 연구에서 수련받는 내시경 의사들을 교육하여 원주상피로 대체된 식도의 진단율을 높였다는 연구 결과가 있다[62]. 그러므로 원주상피로 대체된 식도를 인식하고 조직 검사를 시행하는 것이 바렛식도의 진단율을 높일 수도 있다.

정상 범위 내로 인식되고 있던 1 cm 미만의 원주상피로 대체된 식도에서 적극적으로 조직 검사를 시행하였던 Splechler 연구는 식도 선암이 증가하고 있던 서구의 상황에서 이루어졌기에 일본의 진단 기준과 같이 원주상피로 대체된 식도 길이에 관계없이 바렛식도를 진단하기 위하여 우리나라에서도 조직 검사를 적극적으로 시행하여야 하는가에 대한 명확한 답변은 어렵다. 다만 우리나라에서는 바렛식도에서 기원하는 것으로 알려진 식도 선암의 발생률이 감소 추세에 있고[7], 위식도 접합부 암의 경우에도 1990년도와 비교하여 발생률에 변화가 없는 것으로 보고[63]된 현재 시점에서는 적어도 우리나라에서 바렛식도의 전암 병변으로서의 위험을 침소봉대하지 않는 것이 적절하다고 판단된다.

또한 전국민을 대상으로 하여 2년 간격으로 위암 선별 검사를 시행하고 있는 우리나라에서는 바렛식도의 선별 검사를 위하여 별도의 내시경 검사를 권고할 필요성 역시 낮을 것으로 판단된다. 다만 장분절 바렛식도의 경우 서구의 권고안에 근거하여 치료 방침을 수립하는 것이 근거 있는 타당한 의학적 판단일 것으로 보인다.

본 리뷰를 통해 머지않은 미래에 우리나라에서도 바렛식도 환자의 진단과 치료에 대한 권고안이 수립될 수 있도록 다양한 연구 결과가 나오기를 기대해 본다.

The cardia is defined from the point B, where the phreno-esophageal ligament begins, to the point D, above the imaginary line XY. The cardia is lined partly by squamous, and partly by columnar epithelium, and thus the junctional epithelium extends across the esophago-gastric junction.

The key steps are: 1. Identify the gastro-esophageal (GE) junction at the top of the gastric mucosal folds. 2. If hiatus hernia is present, do not confuse the diaphragmatic hiatal impression for the GE junction. 3. For circumferential columnar-appearing mucosa above the GE junction, define the extent in centimeters above the GE junction – report as the C value. 4. For any tongue-like areas of columnar-appearing mucosa, measure the maximum extent, in centimeters, above the GE junction – report as the M value. UI, upper inscisor.
